Devital dişlerin ıntrakoronal ağartmasında kullanılan iki farklı materyalin klinik etkinliğinin karşılaştırılması

Devital diş renklenmeleri diş dizisinde kolayca ayırt edilebildiği için özellikle ön dişlerde estetik sorunlara yol açmaktadır. Bu çalışmada kanal tedavisi uygulanmış veya travmaya maruz kalmış, ön dişlerde ortaya çıkan renklenmelerin ağartılması amacıyla bir sodyum perborat (Starbrite, Dentramar, Hollanda) ve hidrojen peroksit (Opalescence Xtra, Ultradent, ABD)içerikli beyazlatma ajanı kullanılarak 24 hasta tedavi edilmiştir. Vita skalası ve fotoğraf kaydı ile ağartmanın kalitatif değerlendirilmesi yapılmıştır. İki gün aralıklarla sekizinci gün sonunda incelenen ağarma simetrik sağlıklı dişlerin rengine ulaştığında başarılı olarak kabul edilmiştir. Kullanılan iki materyal uygulama süresi ve uygulama kolaylığı açısından da karşılaştırılmıştır. Daha sonra altı aylık aralıklarla 3 yıl boyunca takip edilen hastalarda renkte geriye dönüşün yanı sıra, diş sert dokularında kayıp, çatlak, servikal rezorbsiyon ve radyografik bulguların varlığı incelenmiştir. Opalescence Xtra ile beyazlatma ortalama iki dört gün arasında sağlanmış, ancak Starbrite uygulanan sekiz vakada (% 66, 7) sekizinci gün sonunda yeterli ağartma elde edilmemiştir. Sonuç olarak uygulama süresi ve kolaylığı açısından Opalescence Xtra ile tedavinin avantajlı olduğu saptanmıştır. Radyografik ve klinik olarak dişlerde sert doku kaybıyla ilgili bir bulguya rastlanmamış, ancak bazı vakalarda ağartmada hafif düzeyde geriye dönüş saptanmıştır.

Abstract


Comparison of the clinical efficiancy of two materials used for intracoronal bleaching of devital teeth



Discolorations of nonvital teeth lead to esthetic problems since they can be easily distinguished in teeth row. In this study, 24 patients were treated using a sodium perborate material (Starbrite Dentramar, Netherlands) and hydrogen peroxide (Opalescence Xtra, Ultradent, USA) for intracoronal bleaching of anterior teeth discolorated due to trauma or root canal therapy. Qualitative evaluation was made by using Vita scale and photographic record. Bleaching examined at the end of the eighth day with two-day interval was accepted successful, provided the bleaching attained the color of the symmetrical tooth. Two materials used were compared in terms of handling and application time. Then loss of tooth hard tissues, cracks, cervical resorptions and radiographic findings as well as relapse of color were examined 3 years long with 6-month intervals. Bleaching with Opalescence Xtra was achieved between two-four days. However, in eight cases (%66,7) satisfactory bleaching was not reached with Starbrite application. In conclusion, the treatment with Opalescence Xtra as regards the handling and application time proved advantages. No clinical or radiographic findings of hard tissue loss were determined, but in some cases a slight relapse of color was observed.
