Dental florozisin beyazlatılmasında kullanılan bir Office Bleaching metaryalinin pulpa ve periapikal dokular üzerindeki yan etkilerinin incelenmesi

Özet

Amaç: Önemli düzeyde estetik, hatta psikolojik problemlere yol açan dental florozise bağlı renklenmelerin tedavisinde estetiği kazandırırken dişe en az zarar verecek bir tedavi yönteminin seçilmesi amaçlanmalıdır. Günümüzde, beyazlatma yöntemlerinin protetik yaklaşımdan daha konservatif olduğu düşünülmektedir. Bu çalışmada amaç, ağartma işlemleri sırasında oluşan kimyasal reaksiyonların pulpaya veya dişin sert dokusuna yan etkilerinin klinik olarak incelenmesidir Gereç ve Yöntem: SDÜ Diş Hastalıkları ve Tedavisi AD Kliniğinde, 2- 4 yıl önce ofis beyazlatma işlemi (Opalescence Xtra, Ultradent, USA) uygulanmış ve estetik başarı sağlanmış, 15 dental florozisli hastanın toplam 184 (92 beyazlatılan ve 92 kontrol) dişi, vitalite kaybı, servikal rezorpsiyon, radyografik bulgu ve hipersensitivite varlığı açısından incelendi. Bulgular: 92 dişten 12.sinde (%13) çeşitli düzeylerde hipersensitivite varlığı gözlenirken radyografik olarak herhangi bir patoloji gözlenmedi. Beyazlatma yapılan dişler ve kontrol dişleri arasında vitalite bulguları açısından istatistiksel olarak anlamlı düzeyde bir farklılık saptanmadı. Radyografik ve klinik kontrollerde servikal rezorpsiyona rastlanmadı. Sonuç: Geçici ve tolere edilebilir düzeydeki hipersensitivite, pulpanın reversibl olarak etkilendiğini düşündürmektedir. Vitalite kaybının olmaması da bunu destekler niteliktedir.
Uygulanan ağartma yönteminin hipersensitivite varlığı dışında herhangi bir yan etkisinin gözlenmemesi güvenli bir yöntem olduğunu göstermektedir.

Abstract

Investigation of the side effects of an office bleaching technique on pulpal or periapical tissues

Aim: Discoloration of the teeth induced by dental fluorosis, result in esthetic and even psychological problems.It should be aimed to choose the less harmful and the most esthetic treatment method. Today, bleaching procedures are thought to be more conservative than prosthetic approaches. The purpose of this study was to find out the effects of chemical reactions on pulp and hard tissues of the tooth during bleaching procedures. Materials- methods: Hundred eighty four (92 bleached and 92 controls) teeth of 15 patients with dental
fluorosis who had been treated before 2-4 years with office bleaching (Opalescence Xtra, Ultradent, USA) procedures in Faculty of Dentistry of SDU, Department of Restorative Dentistry and Endodontics and esthetic success was achieved had been evaluated in respect to vitality loss, cervical resorbtion, radiographic findings and hypersensitivity. Results: Twelve out of 92 teeth (% 13) had hypersensitivity at variable levels, but no radiographic pathology. There was no significant difference between bleached teeth and controls in respect to vitality
