THE RELATIONSHIP BETWEEN OBESITY AND SKELETAL AND DENTAL DEVELOPMENT IN ADOLESCENTS

Abstract

Objective
This study aims to compare the growth and
development of obese and normal weight individuals
in terms of craniofacial development, dental age and
cervical vertebral maturation (CVM).
Material and Method
Among the patients aged 12-16 who applied to the
clinic for orthodontic treatment, 51 obese (31 female,
20 male) patients with a body mass index at or
above the 95th percentile and 52 normal weight (32
female, 20 male) patients between the 16th and 85th
percentile were included in the study. The craniofacial
development, cervical vertebral maturation and
dental age of the individuals were evaluated from
the cephalometric and panoramic radiographs taken
at the beginning of the treatment. Since parametric
assumptions were not provided, the Mann-Whitney U
test was used for comparison between groups.
Results
The mean values of female patients’ SNA, SNB,
Y-Axis angle and Pm-A showed statistically significant
differences between the groups (p<0.05). Obese
females had a higher mean for the SNA, SNB and
Pm-A variant, while females in the control group had
a higher mean for the Y-Axis angle variable. The
mean of males’ SNA, SNB and N-ANS variables
showed a statistically significant difference between
the two groups (p<0.05). Males in the obese group
have higher mean SNA and SNB values than those
in the control group. Males in the control group had
a higher mean N-ANS value than the obese group.
A moderate, inverse and significant relationship was
found between age, dental age and CVM (p<0.05).
As a result of the regression analysis, while the
relationship between chronological age, dental age
and obesity was found to be statistically significant
(p<0.05), the relationship between CVM and obesity
was not statistically significant (p>0.05).
Conclusion
Craniofacial morphology shows some differences
between obese and normal weight adolescents.
Orthodontists should consider craniofacial
morphology and dental development characteristics
when planning treatment in obese individuals.

ADOLESAN BİREYLERDE İSKELETSEL VE DENTAL GELİŞİMİN OBEZİTE İLE İLİŞKİSİ

Abstract

Amaç
Bu çalışmada obez ve normal kilolu bireylerin kraniyofasiyal
gelişim, dental yaş ve servikal vertebra maturasyonu
(CVM) açısından büyüme ve gelişimlerinin
karşılaştırılması amaçlanmıştır.
Gereç ve Yöntem
Ortodontik tedavi için kliniğe başvuran 12-16 yaş aralığındaki
hastalardan vücut kitle indeksi persentil değeri
≥%95 olan 51 obez (31 kız, 20 erkek) hasta ile
persentil değeri %16-85 olan 52 normal kilolu (32 kız,
20 erkek) hasta çalışmaya dahil edildi. Tedavi başında
alınan sefalometrik ve panoramik radyografilerden
bireylerin kraniyofasiyal gelişimi, servikal vertebra
maturasyonu ve dental yaşı değerlendirildi. Parametrik
varsayımlar sağlanmadığı için gruplar arası karşılaştırmada
Mann-Whitney U testi kullanıldı.
Bulgular
Kızların SNA, SNB, Y aksı açısı ve Pm-A değerlerine
ait ortalamalar gruplar arasında istatistiksel olarak
anlamlı farklar göstermiştir (p<0.05). SNA, SNB ve
Pm-A değişkeni için obez kızlar daha yüksek ortalamaya
sahipken, Y aksı açısı değişkeni için kontrol
grubundaki kızlar daha yüksek ortalamaya sahiptir.
Erkeklerin SNA, SNB ve N-ANS değişkenlerine ait
ortalamaları iki grup arasında istatistiksel anlamlı bir
fark göstermiştir (p<0.05). Obez gruptaki erkeklerin
SNA ve SNB değerleri kontrol grubunda olanlara göre
daha yüksek ortalamaya sahiptir. N-ANS değeri için
ise kontrol grubundaki erkekler obez gruba göre daha
yüksek ortalamaya sahiptir. Yaş, dental yaş ve CVM
arasında orta şiddetli aynı yönlü, anlamlı bir ilişki bulunmuştur
(p<0.05). Regresyon analizi sonucunda
kronolojik yaş ve dental yaş ile obezite arasındaki
ilişki istatistiksel olarak anlamlı bulunurken (p<0.05),
CVM ile obezite arasındaki ilişki istatistiksel olarak
anlamlı değildir (p>0.05).
Sonuç
Kraniyofasiyal morfoloji, obez ve normal kilolu adolesan
bireyler arasında bazı farklılıklar göstermektedir.
Ortodontistler obez bireylerde tedavi planlaması yaparken
kraniyofasiyal morfoloji ve dental gelişim özelliklerini
göz önünde bulundurmalıdır.
