Diş Çekimini Takiben İmmediat Olarak Yerleştirilen İmplantların Sağ Kalımını Etkileyen Faktörlerin Değerlendirilmesi: Retrospektif Çalışma

Abstract

Amaç: Bu çalışmanın amacı, diş çekimini takiben immediat olarak yerleştirilen implantların sağ kalımının ve sağ kalımı etkileyen olası risk faktörlerinin retrospektif olarak değerlendirilmesiydi.

Gereç ve Yöntemler: Çalışmaya, 2015 Ocak ve 2021 Haziran tarihleri arasında, en az bir adet diş çekimi yapılmış ve çekim soketine eş zamanlı olarak implant yerleştirilmiş hastalar dahil edilmiştir. Demografik özellikler (yaş, cinsiyet ve sistemik hastalık) ve ameliyat verileri (dişin çekilme nedeni, implantın yerleştirildiği diş bölgesi, implantın yüzey özelliği, boy ve çapı, yerleştirme tork değeri, implant sayısı, kemik doku greftleme ve flep elevasyonu) kaydedilmiştir. İmplant sağ kalım oranı, toplam yerleştirilen implant sayısına göre yüzdelik olarak hesaplanmış ve risk grupları üzerindeki etkileri belirlemek için Multivariate analizi yapılmıştır. İstatistiksel olarak anlamlılık p<0,01 ve p<0,05 düzeylerinde değerlendirilmiştir.

Bulgular: Toplam 69 hasta (41 kadın ve 28 erkek; yaş aralığı 20 ile 84; yaş ortalaması 51,33±15,49) ve 109 implant değerlendirmeye alınmıştır. İmplant sağ kalım oranı %90,8 olarak hesaplanmış ve implantlar, ameliyat sonrası 12 aylık dönemde kaybedilmiştir. İmplant kaybı ile implantın yerleştirildiği diş bölgesi, flep elevasyonu ve yerleştirme tork değeri arasında istatistiksel olarak anlamlı bir fark bulunmuştur (sırasıyla p=0,004, p=0,001 ve p=0,001).

Sonuçlar:Tedavi planlamasının olası risk faktörleri dikkate alınarak yapılması, immediat olarak yerleştirilen implantların klinik başarısının arttırılması için oldukça önemlidir.

Evaluation of the Factors Affecting the Survival of Implants Placed Immediately Following Tooth Extraction: A Retrospective Study

Abstract

Background: The purpose of this study was to retrospectively evaluate the survival and its affecting factors for implants placed immediately following tooth extraction.

Methods: This study was conducted in patients who underwent at least one tooth extraction and implant placement at the same time, between January 2015 and June 2021. Demographic (age, gender, and co-morbid conditions) and surgical data (reasons for tooth extraction, site of placed implant, surface characteristics, length and diameter of the implant, insertion torque, number of implants, bone grafting and flap elevation) were recorded. Implant survival rate was calculated as a percentage of the total implant number and multivariate analysis was performed to determine the effects on risk groups. Statistical significance was set at p<0.01 and p<0.05.

Results:A total of 69 patients (41 women and 28 men; mean age 51.33±15.49 years) and 109 implants were enrolled. The survival rate of implants was calculated as 90.8%, and the implant failure was observed at the 12-months of follow-up. There was a statistically significant difference between the implant failure and the site of placed implant, flap elevation and insertion torque (p=0.004, p=0.001 ve p=0.001, respectively).

Conclusion:It is important that treatment plan is consider with the possible risk factors to increase the clinical success of the implants placed immediately.
