Çocuklarda glukoz -6- fosfat dehidrogenaz enzim eksikliği: 2 olgu sunumu

Abstract

Amaç: Glukoz-6-fosfat
dehidrojenaz (G6PD) enzim eksikliği; insanlarda en yaygın görülen kırmızı kan
hücresi defektidir ve dünya üzerinde; başta Akdeniz ülkelerinde olmak üzere
yaklaşık 400 milyon insanı etkilediği düşünülmektedir. Bu enzim defektine sahip
bireylerde bazı ilaçların kullanılması ve fava fasulyesinin tüketilmesi kırmızı
kan hücrelerinin oksidatif strese karşı tamir gücünü azaltarak, hemolize neden
olmaktadır. Bu durumun en sık görülen klinik belirtileri; akut ve kronik
hemoliz, yenidoğan hiperbilirubinemisidir ancak bazı durumlarda hasta klinik
olarak asemptomatik olabilir.



            Bu enzim
eksikliğine sahip bireylerin diş hekimliği açısından önemi; hemolizin
enfeksiyonla tetiklenmesi ve kullanılan prilokain, lidokain gibi lokal
anesteziklerin ve bazı antimikrobiyal ajanların hemolize neden olmasıdır. Bu
olgu raporunda G6PD enzim defektine sahip 2 çocuk hastanın
klinik,
radyolojik, intraoral bulgular ile dental tedavi yaklaşımları sunulmuştur.
