Türk popülasyonunda taurodontizm prevalansinin radyografik olarak incelenmesi

Abstract

Amaç: Taurodontizm, dişin anatomik yapısıyla karakterize gelişimsel bir anomalidir. Dişin yapısal anomalilerinin en önemlilerinden biri olan taurodontizm, radyografide vertikal olarak uzamış pulpa odası, apikale doğru yer değiştirmiş pulpa tabanı ve iki ya da üçe ayrılmış normale göre kısa kök/kanal yapıları ile kendini göstermektedir. Taurodont dişler köklerin sayısı, şekli ve karmaşık anatomisi nedeniyle anormal kök kanallarına sahiptir. Bu dişlere endodontik tedavi uygulanırken, biyomekanik preparasyon ve kök kanal doldurma işlemleri sırasında zorluk yaşanabilir. Bu çalışmanın amacı Kütahya Sağlık Bilimleri Üniversitesi Diş Hekimliği Fakültesi’ne başvuran hastalar arasında taurodontizm prevalansını belirlemek, lokalizasyonu, dağılımını, cinsiyetler arasındaki farklılığını incelemektir.
Gereç ve Yöntemler: Bu çalışmada çeşitli dental problemler nedeniyle Kütahya Sağlık Bilimleri Üniversitesi Diş Hekimliği Fakültesi’ne başvuran 500 hastanın dijital panoramik radyografları taurodontizm açısından retrospektif olarak incelendi. Taurodontizmin tiplerinin görülme sıklığı ile cinsiyet ve çeneler açısından görülme oranları değerlendirildi. Elde edilen verilerin istatistiksel analizi Pearson ki-kare testi ile yapıldı.
Bulgular: Çalışmada toplam 500 adet panoramik radyografi (279 erkek, 221 kadın) değerlendirildi. Bu radyografların 37 (%7.4)’ünde ve incelenen 4126 molar dişin 56 (%1.4)’ünde taurodontizm anomalisi saptandı. Bu dişlerin 26 (% 46.4) tanesi hipotaurodont, 10 (%17.9) tanesi mezotaurodont, 7 (%12.5) tanesi hipertaurodont ve 13 (% 23.2) tanesi piramidal diş olarak sınıflandırıldı. Taurodont dişlerin 37 (%66.1)’inin mandibulada, 19 (% 33.9)’unun maksillada olduğu görüldü. Mandibulada taurodontizm prevalansının maksillaya oranla daha yüksek olduğu tespit edildi (p<0.05).
Sonuç: Taurodontizm yaygın olarak görülen klinik bir anomalidir. Hekimler molar dişlere endodontik tedavi planlarken, bu dişlerde görülebilecek taurodontizm anomalisi açısından dikkatli olmalıdır.
