Percutaneous Endoscopic Gastrostomy; Experience of two clinics

Abstract

Objective: Percutaneous endoscopic gastrostomy (PEG) is the most common and most simple, minimally invasive method in gastrostomy methods. So, The aim of the study was to evaluate at the result of patients who underwent PEG at our clinics. At the same time, we was to estimate the influence of PEG placement on gastro-esophageal reflux.Material and methods: Between January 2010 and 2012, 13 patients who underwent PEG tube in two pediatric surgery centers were retrospectively evaluated.Results: There were 7 female (%54), 6 male (%46) patient. The mean age was 3,5. The mean duration of hospital stay was 3 ± 1 days. The mean time to start feeding was 31 ± 8 hours, the mean operation time was 45 ± 15 minutes. There was no significant difference in feeding time between girls and boys. Indication for PEG insertion was swallowing dysfunction due to neurological condition in 12 patients and swallowing dysfunction after repair of esophageal atresia in 1 patient. We have performed gastroesophageal reflux analysis before in our patients. Two patients with hypoxic ischemic encephalopathy who had aspiration pneumonia after PEG insertion and one patients who underwent esophageal atresia repair, had undergone antireflux surgery. There aren't postoperative complication in other ten patients and they are feeding with PEG.Conclusion: PEG is a technically feasible method. PEG may increase the rate of gastroesophageal reflux. PEG could be performed without invasive analysis and surgery. Invasive analysis and surgery may be done later.

Perkütan endoskopik gastrostomi; İki kliniğin deneyimi

Abstract

Amaç: Perkütan endoskopik gastrostomi ( PEG ) gastrostomi yöntemleri içinde en sık ve en basit uygulanan minimal invaziv yötemdir. Çalışmamızda PEG uygulanan hastaların sonuçları ve reflü hastalığı ile ilişkisi değerlendirildi.Gereç ve Yöntem: Ocak 2010 – Ocak 2012 tarihleri arasında iki çocuk cerrahisi merkezinde perkutan endoskopik gastrostomi kateteri takılan 13 hastanın dosyası geriye dönük incelendi.Bulgular: Hastaların 6'sı erkek ( % 46 ), 7'si kız ( % 54 ) idi. Yaş ortalaması 3,5 yıl idi. Hastaların ortalama hastane yatış süresi 3 ± 1 gündü. Hastaların beslemeye başlama süreleri ortalama 31 ± 8 saat, operasyon süreleri ise ortalama 45 ± 15 dakika idi. Kız ve erkek hastalarda beslemeye başlama süreleri açısından fark yoktu. PEG takılma endikasyonları 1 hastada özefagus atrezisi onarımı sonrası yutma disfonksiyonu gelişmesi ve 12 hastada da santral sinir sistemini patolojilerine bağlı yutma disfonksiyonuydu. Olgularımızda başlangıçta gastroözofagial reflü araştırması yapıldı. Hipoksik iskemik ensefalopati tanılı, PEG sonrasında sık aspirasyon pnömonisi geçiren 2 hastaya ve özefagus atrezisi onarımı yapılmış 1 hastaya daha sonra antireflü ameliyatı yapıldı. Diğer 10 hasta operasyon sonrası komplikasyonsuz olarak PEG ile beslenmeye devam etmektedir.Sonuç: PEG işlemi kolay uygulanabilir bir yöntemdir. Herhangi bir nedenle gastrostomi takılması gereken olgularda gastrostomi öncesi invaziv araştırma ve cerrahi yapmadan minimal invaziv yöntem olan PEG yöntemi ile gastrostomi açılıp daha sonra gerekirse detaylı inceleme ve invaziv cerrahi yapılabilir.
