Placental Abruption: Epidemiology, Analysis of Risk Factors, Fetal and Maternal Consequences

Abstract

Aim: To retrospectively investigate characteristics of patients who were operated in two referral clinics with the diagnosis of placental abruption or who had incidentally found to have placental abruption during cesarean section.Material and Methods: We reviewed patient files of 92 patients who were surgically treated with the diagnosis of placental abruption or had incidentally diagnosed with placental abruption in two referral clinics between January 2011 and December 2012. Patient ages, gravidity, parity, gestational week at diagnosis, predisposing factors, fetal and maternal complications were analyzed.Results: Mean patient age was 32,4 years. Mean gravidity and parity were 4,26 and 3,02, respectively. Of the patients investigated, 25% had hypertension, 19,56% had intrauterine growth retardation, 16,30% had premature delivery or early rupture of membranes, 20,65% had non-vertex presentation, 8,7% had polihydramnios, 26,08% had history of smoking, 2,17% had trauma history, 2,17% had twin pregnancy and 7,59% had uterine myoma detected as predisposing factors.11,95% of the patients were admitted to our clinics with hypovolemic shock. We detected disseminated intravascular coagulation in 7,60%, acute renal failure in 8,69%, couvelaire uterus in 14,13% of the patients. Six patients underwent hysterectomy due to uterine atony. One patient died due to severe pulmonary edema. Blood and blood products were administered to 36,95% of the patients. Mean delivery week was 28,25 and mean birth weight was 1588g. Mean first minute APGAR score was 4,32 for the babies born alive. On admission, 22,82% of the patients had intrauterine fetal demise. 19,56% of the fetuses died within 24 hours after delivery. Conclusion: Placental abruption is a clinical entity with high maternal and fetal morbidity and mortality.

Amaç: Çalışmamızda iki referans klinikte dekolman plasenta tanısıyla ameliyat edilen ya da ameliyatta iken dekolman plasenta tespit edilen hastaları retrospektif olarak incelemeyi amaçladık.Gereç ve Yöntem: Ocak 2011-Aralık 2012 tarihleri arasında iki referans klinikte dekolman plasenta sebebiyle ameliyat olan ya da ameliyatta iken dekolman plasenta tanısı konan 92 hastanın dosyası retrospektif olarak incelendi. Hastaların yaşları, gravida ve parite sayıları, gebelik haftaları, predispose faktörler, fetal ve maternal komplikasyonlar incelendi.Bulgular: Hastaların ortalama yaşı 32,4'tü. Hastalarda gravida ve parite ortalamaları sırasıyla 4,26 ve 3,02 olarak bulundu. Hastaların %25'inde kronik hipertansiyon ve/veya preeklampsi, %19,56'sında intrauterin gelişme geriliği, %16,30'unda erken doğum tehdidi ya da erken membran rüptürü, %20,65'inde verteks olmayan geliş, %8,69'unda polihidroamnios, %26,08'inde sigara içimi, %2,17'sinde travma, %2,17'sinde ikiz gebelik ve %7,60'ında myoma uteri predispozan faktör olarak tespit edildi. %11,95 hasta hipovolemik şok ile kliniklerimize başvurdu. %7,60 hastada dissemine intravasküler koagülasyon, %8,69 hastada akut böbrek yetmezliği, %14,13 hastada couvelaire uterus izlendi. 6 hastaya atoni sebebiyle histerektomiye uygulandı. Bir hasta akciğer ödemi sebebiyle hayatını kaybetti. %36,95 hastaya kan ve kan ürünü transfüzyonu yapıldı. Ortalama gebelik haftası %28,25 ve ortalama doğum ağırlığı 1588gr olarak bulundu. Doğan fetüslerde 1. dakika APGAR skoru için ortalama 4,32 idi. %22,82 hastada başvuruda intrauterin ölü fetüs durumu mevcuttu. %19,56 fetüs doğduktan sonra ilk 24 saatte öldü. Sonuç: Dekolman plasenta yüksek maternal ve fatal morbidite ve mortaliteye sebep olan klinik bir durumdur.
