Evaluation of relation between hepatic iron deposition and liver histopathology in patients with Chronic hepatitis c

Abstract

Objective: The aim of this study is to search the effects of hepatic iron deposition for the liver fibrosis and inflammation in patients with chronic hepatitis C (CHC).Materials and Methods: The records of 70 patients with CHC who had not previously been treated with antiviral therapy were reviewed retrospectively. The liver biopsies of these patients have been scored in terms of inflammation and fibrosis with the method of modified ISHAK by only one patholog. Preparations were prepared from these liver biopsies were stained with Persl' Prussian blue. Hepatic iron deposition was evaulated using the method of semicantitative modified MacSween by the same pathology.Results: In the histopathological evaluation of liver biopsy samples, increased hepatic iron deposition has been found in 25 patients (38.5%). A significant relation has been found between increased hepatic iron deposition and histological activity index (HAI) (p=0.001), but not found fibrosis degree (p>0.05). Fibrosis and HAI degrees were higher in patients with severe-moderate hepatic iron deposition.In conclusion: Increased hepatic iron deposition may be associated with severe hepatic fibrosis and inflammation in patients with chronic hepatitis C.

Kronik hepatit c hastalarında karaciğer demir birikimi ile karaciğer histopatolojisi arasındaki ilişkinin değerlendirilmesi

Abstract

Amaç: Kronik hepatit C (KHC) hastalarında karaciğer demir birikiminin karaciğer fibrozisi ve enflamasyonu üzerine etkilerini araştırmak.Gereç ve Yöntemler: Daha önce antiviral tedavi almamış 70 KHC'li hastanın kayıtları retrospektif olarak tarandı. Bu hastaların karaciğer biyopsileri tek bir patolog tarafından modifiye ISHAK yöntemiyle enflamasyon ve fibrozis açısından skorlandı. Bu karaciğer biyopsilerinden tekrar hazırlanan preparatlar Persl' Prusya mavisi ile boyandı. Karaciğerdeki demir miktarı semikantitatif Modifiye MacSween karaciğer demir yükü evrelemesi kullanılarak aynı patolog tarafından değerlendirildi.Bulgular: Karaciğer biyopsi örneklerinin histopatolojik olarak incelenmesinde 25 hastada (% 35.7) artmış karaciğer demir birikimi tespit edildi. Artmış karaciğer demir birikimi ile histolojik aktivite indeksi (HAI) arasında anlamlı bir ilişki varken (p=0.001), fibrozis ile anlamlı ilişki saptanmadı (p>0.05). Ağır-orta artmış karaciğer demir birikimi olan olgularda fibrozis ve HAI dereceleri daha ileri idi.Sonuç: KHC'li hastalarda, artmış karaciğer demir birikimi ilerlemiş karaciğer fibrozisi ve enflamasyonu ile ilişkili olabilir.
