Çocuklarda Göğüs Ağrısına Yaklaşım

Abstract

Göğüs ağrısı çocuk acile başvuruların en sık nedenlerinden biridir. Erişkinlerin aksine kardiyak nedenli göğüs ağrıları çocuklarda oldukça nadir olmasına rağmen, çocuk kardiyoloji konsültasyonlarının önemli bir kısmını oluşturur. Ciddi ölüm kaygıları nedeni ile bu hastalara gereksiz, uzun süren ve maliyeti yüksek tetkikler yapılmaktadır. Dikkatle detaylı alınmış bir öykü ile desteklenen fizik muayene ve elektrokardiyografide (EKG) anormal bulgusu olmayan hastalarda kardiyak nedenler büyük olasılıkla dışlanabilmektedir. Akut başlangıçlı ve egzersizle ilişkili olmayan, senkop ya da baş dönmesinin eşlik etmediği, aile öyküsü negatif bulunan, fizik muayene ve EKG’ sinde patolojik bulgu saptanmayan hastaların ekokardiyografi ve egzersiz testi gibi daha ileri tetkik amacıyla çocuk kardiyolojiye gönderilmesi hasta ve ailesinde gereksiz endişeye ve aşırı maliyete neden olmaktan öteye geçmeyecektir.

Approach to Children with Chest Pain

Abstract

Chest pain is one of the most common complaint in children admitting the emergency department. Cardiac induced chest pain are seen rarely in children compared to adults, despite this it constitutes an important part of paediatric cardiology consultations. Many unnnecessarily high cost tests, which take long time, might be performed due to serious concern of death of the parents. A detailed obtained history which is supported with physical examination and electrocardiography (ECG) without any abnomality can rule out any cardiac cause. It might cause unnecessary concern to refer to paediatric cardiologist for further testing for both of patients and their parents and high cost for patients who have acute onset pain unrelated to exercise and negative family history without any dizziness or syncope and without any abnormal findings on physcial examination and ECG.
