Sol alt kadran ağrısında ayırıcı tanı: Situs inversus totalisli hastada akut apandisit

Abstract

Karın ağrısı acil servise başvurularda en sık görülen şikayetlerden biridir. Abdominal ağrı ile başvuran hastalarda görülen en sık cerrahi gerektiren hastalık apandisittir. Sol alt kadran ağrısı ile başvuran hastalarda ayırıcı tanıda akut apandisit ilk planda akla gelmemektedir. Bu durumun oluşmasında yetersiz muayene ve eksik anamnez alınmasının yanlış negatif tanı oranını yükselttiği kabul edilmektedir. Solda yerleşimli apandisit situs inversus totalis veya karın içi organların malrotasyonu ile birlikte görülür. Sunumda sol taraflı akut apandisit tanısı almış bir situs inversus totalis olgusu değerlendirilecektir.

Differential diagnosis in left lower quadrant pain: Acute appendicitis in patients with situs inversus

Abstract

Appendicitis is the condition that most frequently requires surgery on patients presenting with abdominal pain. In situs inversus cases, appendicitis is difficult to diagnose if no situs inversus totalis diagnosis was previously made since appendix is in an abnormal position. When appendicitis diagnosis is delayed in such cases, complications may arise such as perforation followed by abscess formation which would deteriorate the clinical progress of the patient and make the operation difficult. This paper reminds that the appendicitis should be included in differential diagnosis in patients presenting with left lower quadrant pain by keeping situs inversus in mind
