Acil Abdominal Cerrahide Ortalama Trombosit Hacmi: Literatür Taraması

Abstract

Amaç Bu çalışmada, acil abdominal cerrahi gerektiren hastalıklarda ortalama trombosit hacminin incelendiği çalışmaları değerlendirerek, bu parametrenin tanısal ve prognostik değerini araştırmayı amaçladık. 


Materyal ve Metot: Ocak 2010 – Aralık 2014 tarihleri arasında yayınlanmış makaleleri saptamak amacıyla “mean platelet volume” anahtar kelimeleri kullanılarak, “http: //www.ncbi.nlm.nih.gov/pubmed/” internet sitesinden tarama yapıldı. Elde edilen 646 orijinal makale yayınlandığı yıla, ülkeye, konusuna, türüne, bilim dalına göre değerlendirildi. Acil abdominal cerrahi hastalıklar ile ilgili 12 makale belirlendi, bunlarda ayrıca konuları, nitelikleri (tanısal, prognostik) ve istatistiksel anlamlarına göre değerlendirildi.

 
Bulgular: Ortalama trombosit hacmi ile ilgili son iki yılda yayınlanan makale sayısı 431 (%66.71), son beş yılda Türkiye den yayınlanan makale sayısı 354 (%54.79), Dünya genelinde Kardiyoloji kliniklerinden yayınlanan makale sayısı 199 (%30.8) olup, bu değerler anlamlı şekilde yüksek olarak belirlendi. Acil Tıp kliniklerinden sadece 11 (%1.70) makale yayınladığı belirlendi. Acil abdominal cerrahi hastalıklar ile ilgili çalışmalar tamamı Türkiye’den yapılmış retrospektif karakterde yayınlardı. Bu çalışmaların tümünde kontrol guruplarının benzer yaş aralığında sağlıklı bireylerden oluşturulduğu belirlendi. 


Sonuç: Acil abdominal cerrahi hastalıklarda, ortalama trombosit hacminin inflamasyona karşı farklı yanıtlar geliştirdiği gösterilmiş ve çalışılan diğer inflamatuar belirteçlere üstünlüğü saptanamamıştır. Yapılan çalışmaların tümünde taraf tutma söz konusu olabilir. Ortalama trombosit hacminin akut abdominal cerrahi hastalıklarda, tanısal veya prognostik değerini belirleyebilmek için prospektif çalışmalara ihtiyaç vardır.
