TUR-P Sonrası Üretral Kateter Çekim Süresinin Operasyon Sonrası İşeme Parametreleri Üzerine Etkisi

Abstract

Amaç: Bu çalışmada Transüretral prostat rezeksiyonu (TURP) sonrası kateter çekim süresinin ameliyat başarısı üzerine etkisinin değerlendirilmesi amaçlanmıştır.

Yöntem: TUR-P yapılan hastalar, kateter çekim sürelerine göre 3 gün altı ile 3 günden fazla olmak üzere randomize olarak iki gruba ayrıldı. Her iki grupta bulunan hastaların yaş, preoperatif total PSA, Hb, Qmax, PVR, ameliyatta çıkarılan doku miktarı, post operatif Hb, Qmax, PVR değerleri kaydedildi. Preoperatif ve postoperatif Omax değerleri arasındaki değişim gruplar arasında hesaplandı. Parametrik veriler ortalama ± standat sapma, parametrik olmayan veriler min - maks olarak verildi. Grup verilerinin karşılaştırılmasında Mann Whitney U test kullanıldı.

Bulgular: Grup 1’de 37, grup 2’de 45 hasta vardı. Hastaların yaş ortalamaları grup 1’de 66.8 ± 5.0 yıl, grup 2’de 66.8 ± 8.4 yıl idi. Grupların PSA, Hb, Htc, Q maks, PVR, çıkan doku miktarları ve operasyon sonrası Q maks değişimleri Tablo 1’de özetlendi. Yaş, PSA, Hb, Htc, Qmaks, PVR verileri arasında guplar arasında anlamlı fark görülmedi. Operasyon sonrası Qmax değişimi (Post op Q maks- Preop Q maks) ve PVR verileri grup 2 de daha iyi görünmesine rağmen iki grup arasında istatistiksel olarak anlamlı bir fark görülmedi. Fakat kateter süresi uzun olan hastaların operasyonda çıkarılan doku miktarının istatistiksel olarak anlamlı daha yüksek olduğu dikkati çekti.

Sonuç: TUR-P operasyonlarında postoperatif Qmaks değişimi ve PVR göz önüne alındığında kateter çekim süresinin bu parametrelere anlamlı bir etkisinin olmadığı gözlemlendi. ( Sakarya Tıp Dergisi 2016, 6(2):56-59 )
