Yüksek Eritrosit Sedimentasyon Hızının 25-Hidroksi Vitamin D ve C- Reaktif Protein Düzeyleri İle İlişkisi

Abstract

Amaç: Bu kesitsel
tanımlayıcı çalışmada, yüksek eritrosit sedimentasyon hızının (ESH)   25-OH
vitamin D ve C- reactive protein (CRP) düzeyleri ile ilişkisinin belirlenmesi
amaçlanmıştır. 



 Gereç
ve Yöntemler:
ESH ≥ 100 milimetre/saat olup tanı ve
tedavisi sürdürülen 104 hastanın verileri analiz edildi. Hastalar 25-OH vitamin
D düzeyleri < 10, 10-19, 20-29 ve ≥ 30
ng/mL olarak 4 guruba bölündü. Bu guruplar ile hastaların yaşı, cinsiyeti,
ESH, diyabet varlığı, CRP, lökosit sayısı, albümin ve diğer biyokimyasal ve
hormonal parametreler ile karşılaştırıldı.



Bulgular: Solunum
sisteminin malin olmayan hastalıkları % 37,5 ile en fazla oranda idi. Hastaların
yaş aralığı 25-92, 25-OH vitamin D düzeyi < 30 ng/mL olanlar % 92,3, CRP’si
yüksek olanlar % 97,1 idi. Gruplar arasında ESH, CRP, yaş, cinsiyet, diyabet
varlığı, albümin, lipid profili ve ferritin düzeyleri bakımından anlamlı fark
bulunmadı. ESH ile albümin (r = - 0,202, p= 0,044) ve ESH ile hemoglobin
arsında negatif korelasyon (r= - 0,404, p=0,001) saptandı.



Sonuç: Yüksek
ESH düzeylerinin hem 25-OH vitamin D hem de CRP düzeyleri ile bir ilişkisinin
saptanmaması çalışmanın tipinden kaynaklanabilir. Bununla birlikte vakaların
çoğunluğunda optimal seviyelerin altında 25-OH vitamin D ve yüksek CRP düzeylerinin
saptanması, 25-OH vitamin D’nin ESH yüksekliğine neden olan inflamatuar
patogenez ile ilişkisi olduğunu düşündürebilir.
