Ameliyathane Dışı Anestezi(ADA) Uygulanan Endobronşial Ultrasonografi (EBUS) İşlemlerinde Premedikasyon İlaçlarının Entegre Pulmoner İndeks Skoruna Etkisi

Abstract

Amaç: Ameliyathane Dışı Anestezi(ADA) uyguladığımız Endobronşial Ultrasonografi (EBUS) işleminde hastaların Akciğer hastalıkları tanısı olması ve girişim yapılacak bölge nedeniyle solunum parametrelerinin değerlendirilmesi, solunumsal komplikasyonların en aza indirgenmesi önem arzetmektedir.Çalışmamızda ADA ile birlikte EBUS uygulanan hastalarda farklı premedikasyon ilaçlarının Entegre Pulmoner İndeks skoruna etkisini araştırmayı amaçladık.
Gereç ve Yöntemler: Ekim 2019- Mart 2020 tarihleri arası hastanemiz Bronkoskopi ünitesinde sedatize edilen hastaların kayıtları retrospektif olarak incelendi.Hastalar Grup 1; EBUS girişiminden 30 dakika(dk) önce 2gr Mg sülfat ampul (%15) /100mL izotonik NaCl içinde 15dk süre ile intravenöz infüzyon + İpratropium bromür nebul (0,5mg/2,5mL) Grup 2; İpratropium bromür nebul (0,5mg/2,5mL) olarak 2 gruba ayrıldı.EtCO2,Solunum Sayısı(SS),SpO2,Kalp Hızı(KH),Ortalama Kan Basıncı(OKB) ve Entegre Pulmoner İndeks düzeyleri premedikasyon öncesi, EBUS işlem başlangıç, EBUS işlem sedasyon sonrası 5.dk, 10.dk, işlem bitiş dk’sı olarak değerlendirildi.
Bulgular: Her iki grup demografik veriler açısından benzerdi(p>0,05).Grup1’in EBUS başlangıç Entegre Pulmoner İndeks değerleri (4,92±2) Grup2 (7,19±2)’ye göre istatistiksel anlamlı olarak daha düşüktü(p=0,000). Grup1 ve 2’nin EtCO2, SS, SpO2, KH, OKB ve Entegre Pulmoner İndeks değerleri karşılaştırıldığında istatistiksel olarak anlamlı fark saptanmadı(p>0,05).
Sonuç: ADA uygulanan EBUS işlemi gerçekleştirilen hastalarda premedikasyon ilacı olarak Mg sülfatın eklendiği Grup 1’le Grup 2’i karşılaştırıldığında Entegre Pulmoner İndeks ortalama değerlerinde bazı parametrelerde anlamlı fark bulunmuştur.Bu sonucun EBUS işlemlerinde daha farklı premedikasyon ilaç kombinasyonlarının uygulandığı protokollerle, kronik hastalık öyküsü olan hastaların incelendiği ayrıca sigara kullanımı gibi faktörlerin de randomize edildiği prospektif, kontrollü klinik çalışmalar açısından faydalı olabileceğini düşünmekteyiz.
