Polikistik Over Sendromlu Kadınlarda İnsülin Direnci ile Serum Copeptin Düzeyi Arasındaki İlişki

Abstract

ÖZET
Amaç: Polikistik over sendromlu (PKOS) kadınlardaki insülin direnci ile serum copeptin düzeyi arasındaki olası ilişkiyi ortaya koymaktır.
Yöntem: Benzer yaş ve vücut kitle indeksine (VKİ) sahip 44 PKOS’lu ve 44 sağlıklı kadın çalışmaya dahil edildi. Kadınların demografik, klinik ve laboratuvar bilgileri dosyalarından elde edildi. Adetin 2-4. günleri arası alınan örnekte serum copeptin düzeyi ölçüldü. PKOS ve sağlıklı kadınların verileri karşılaştırıldı. Serum copeptin düzeyinin anlamlı çıkan değişkenlerle korelasyonu değerlendirildi.
Bulgular: PKOS’lu kadınlarda, klinik açıdan infertilite, hirşutizm, akne, ciltte yağlanma ve saç dökülmesi daha fazla, Ferriman-Gallwey Skoru (FGS) daha yüksek, adetler arası süre daha uzundu (bütün değişkenlerde p <0,05). PKOS’lu kadınlarda; LH, Serbest testesteron, toplam testesteron, DHEA-SO4, açlık insülin, HOMA-IR ve copeptin düzeyleri ile insülin direnci oranı yüksek saptandı (bütün değişkenlerde p <0,05). Serum copeptin düzeyi azalan sırayla HOMA-IR, açlık insülin, AMH, toplam testesteron, serbest testesteron ve LH düzeyiyle pozitif korele saptandı (sırasıyla p1:0,728, p2:0,692, p3:0,440, p4:0,431, p5:0,378, p6:0,359).
Sonuç: PKOS’lu kadınlarda serum copeptin düzeyi yüksektir. Serum copeptin düzeyi başta insülin direnci belirteçleri olmak üzere AMH ve androjenlerin düzeyiyle de koreledir.
