SAĞLIK DÜZEYİ GÖSTERGELERİ AÇISINDAN OECD ÜLKELERİNİN SIRALAMASI VE GELİR-SAĞLIK HARCAMALARI ETKİSİ

Abstract

Sağlık düzeyi göstergelerinin analizi, istenilen hedeflerin yerine getirilmesi konusunda politika yapıcılara ve sağlık hizmeti sunucularına önemli bilgiler sunmaktadır. Sağlık sistemlerinin performanslarının uluslararası karşılaştırılmasına yönelik yapılan bu analizler, sağlık politikalarında rol oynayan farklı aktörlerin dikkatini çeken bir konu olmuştur. Bu çalışmada, belirli sağlık düzeyi göstergelerine göre OECD’nin yayınlamış olduğu “Health at a Glance: Europe, 2018” ve “Health at a Glance: Europe, 2019” adlı raporlarda yer alan ülkelerin mevcut sağlık düzeyi ortaya konulmuştur. Bu yolla belirli sağlık düzeyi göstergeleri kullanılarak ülkeler arasında karşılaştırmalar yapılmaya çalışılmıştır. Çalışmada geliştirilen iki senaryo üzerinden ülkelerin sağlık düzeyi göstergelerinin karşılaştırılması ve sıralanması amacıyla Çok Kriterli Karar Verme (ÇKKV) yöntemlerinden biri olan MOORA yöntemi kullanılmıştır. Analiz sonucunda ülkelerin sağlık düzeyi göstergelerini ifade eden ve ülkeler arasında bir sıralama yapılmasına imkân veren MOORA skoru elde edilmiştir. Çalışmada düşük MOORA skoru yüksek sağlık düzeyini ifade etmektedir. Çalışmada ikinci aşamada ise elde edilen MOORA skoru ile belirlenen sosyoekonomik göstergeler arasındaki ilişkinin değerlendirilmesi amaçlanmıştır. Bu amaçla spearman korelasyonu ve regresyon analizi uygulanmıştır. MOORA yöntemiyle elde edilen sonuçlara bakıldığında çalışmada kullanılan sağlık düzeyi göstergeleri açısından en iyi ülkeler senaryo 1 ve 2’ye göre Norveç, İsveç ve İzlanda; kötü olan ülkeler ise Senaryo 1’e göre Türkiye, Letonya ve Estonya; Senaryo 2’ye göre de Macaristan, İtalya ve Letonya şeklindedir. Korelasyon analizi (spearman) sonucunda Senaryo 1’den elde edilen MOORA skoru ile gelir ve sağlık harcamaları arasında negatif yönlü; hava kirliliği ve işsizlik oranı ile ise pozitif yönlü; Senaryo 2’den elde edilen MOORA skoru ile gelir, sağlık harcamaları ve güvenilir içme suyu arasında negatif yönlü; hava kirliliği ve işsizlik oranı ile ise pozitif yönlü bir korelasyon saptanmıştır. Çalışmada yapılan regresyon analizi sonucuna göre, Senaryo 1 ve 2 için bulunan MOORA skoruyla gelir ve sağlık harcamaları arasında negatif yönlü bir ilişki saptanmıştır. Gelir düzeyi ve sağlık harcaması arttıkça MOORA skoru düşmekte ve sağlık düzeyi yükselmektedir.
