Şehir Dışındaki Tedavi Sürecinde Refakatçilerin Hastane Deneyimleri ve Karşılaştıkları Zorluklar

Abstract
Bu araştırma, sağlık kuruluşlarına şehir dışından gelen hastaların refakatçilerinin yaşadıkları deneyimlere ve yaşadıkları zorluklara odaklanmaktadır. Araştırma nitel araştırma yöntemiyle gerçekleştirilmiş olup çalışmaya 10 hasta yakını katılım sağlamıştır. Araştırmanın örneklemi amaca bağlı örneklem yöntemiyle, katılımcı sayısı ise kuramsal örnekleme yöntemiyle belirlenmiştir. Derinlemesine görüşmeler yüz yüze gerçekleştirilmiş olup elde edilen veriler MAXQDA uygulamasında kodlanarak betimsel yöntem ile analiz edilmiştir. Kodların araştırmacı tarafından okunmasıyla 3 farklı temaya ayrıştığı düşüncesi oluşmuştur. Bunlar “hasta yakınlarının süreç içinde tanımladıkları sorunlarla”, “sürece olumlu etki eden faktörlerle” ve “ifade ettikleri çözüm önerileriyle” ilişkilendirilmiştir. Araştırma, tedavi süreçlerinde hastalar kadar onlara bakım veren yakınlarının da birçok anlamda zorluk yaşadığını göstermektedir. Araştırmanın bulguları hasta yakınlarının hastalarına refakat etmek üzere başka bir şehre gitmenin ailenin geri kalanının ihtiyaçlarına yanıt verememe, ekonomik zorluklar, barınma sorunları, psikolojik zorluklar gibi güçlüklere neden olduğunu göstermektedir. Öte yandan bir başka şehirde tedavi görmek hasta yakınları için yabancı bir şehirde ulaşım ve şehre aşina olmamanın getirdiği zorlukları beraberinde getirmektedir. Hasta yakınları hastanede refakat ettikleri süreçte, odaların fiziksel yetersizleri ve sağlık personelinin kullandığı terminolojiyi anlamama gibi zorluklar da yaşamaktadır. Şehir dışındaki bir hastaneden alınan hizmet esnasında hastaların bakımını sağlayan kimselerin yaşadığı sorunların ele alınmasının, hastanın tedavi sürecine de etki edeceği düşünülmektedir. Çalışmaların bu konuya eğilmesinin, farklı sorunları da gündeme getirerek çözüm bulunması noktasında kolaylaştırıcı olabileceği öngörülmektedir. Hastaların ve bakımlarını sağlayan yakınlarının sosyoekonomik ve psikososyal zorluklar yaşadığı da araştırmanın sonuçlarındandır. Dolayısıyla hem politika yapıcılara hem de sağlık kuruluşlarında görev yapan sosyal hizmet uzmanlarına sorunların çözümlenmesi noktasında önemli sorumluluklar düşmektedir.

Abstract
This research focuses on the experiences and difficulties experienced by the companions of patients coming from out of town to health institutions. The research was conducted using the qualitative research method and 10 relatives of patients participated in the study. The study sample was determined using the purposive sampling method and the number of participants was determined using the theoretical sampling method. In-depth interviews were conducted face-to-face and the obtained data were coded in the MAXQDA application and analyzed using the descriptive method. When the codes were read by the researcher, it was thought that they were divided into 3 different themes. These were associated with “problems defined by the relatives of the patients during the process”, “factors that positively affected the process” and “solution suggestions they expressed”. The research shows that the relatives who care for them experience many difficulties as well as the patients during the treatment process. The findings of the research show that the relatives of the patients who go to another city to accompany their patients cause difficulties such as not being able to meet the needs of the rest of the family, economic difficulties, housing problems and psychological difficulties. On the other hand, receiving treatment in another city brings with it the difficulties of transportation in a foreign city and being unfamiliar with the city for the relatives of the patients. During the process in which patients' relatives accompany them in the hospital, they also experience difficulties such as the physical inadequacy of the rooms and not understanding the terminology used by the healthcare personnel. It is thought that addressing the problems experienced by those who provide care for patients while receiving service from a hospital outside the city will also affect the patient's treatment process. Therefore, it is anticipated that studies focusing on this issue may facilitate finding solutions by bringing different problems to the agenda. The results of the study also show that patients and their relatives who provide care experience socioeconomic and psychosocial difficulties. Therefore, both policy makers and social workers working in healthcare institutions have important responsibilities in terms of solving the problems.
