Epstein-Barr Virüs Enfeksiyonunun Nadir Görülen Bir Komplikasyonu: Miyoperikarditli Bir Olgu Sunumu

Abstract

Akut miyokardit genellikle viral enfeksiyonlar sonrası gelişir ve sıklıkla asemptomatik seyreder. Ancak beraberinde perikardit görülebilir. Epstein-Barr virüsü (EBV) enfeksiyonuna bağlı miyoperikardit ise oldukça nadirdir. On altı yaşında erkek hasta şiddetli göğüs ağrısı şikayeti ile başvurdu. Kardiyak enzimleri yüksek saptanan olgunun elektrokardiyografik incelemesinde yaygın elektriksel voltaj düşüklüğü, DII, DIII, aVF, V5, V6 derivasyonlarında ST-T dalga değişiklikleri mevcuttu. Ekokardiyografik değerlendirmede sol ventrikülsistolik fonksiyonları normaldi ve perikardiyal efüzyonu mevcuttu. EBV VCA IgM ve EBV PCR pozitif saptanan hastada EBV miyoperikarditi tanısı düşünüldü. Hasta intravenöz immünglobulin (IVIG) ve nonsteroid anti enflamatuvar tedavisine iyi yanıt verdi ve klinik olarak tam iyileşme görüldü. Göğüs ağrısı ile başvuran hastalarda miyoperikardit ayırıcı tanıda düşünülmeli ve myoperikarditin nadir nedenlerinden EBV enfeksiyonu da akılda tutulmalıdır.

A Rare Complication of Epstein-Barr Virus Infection: A Case Report with Myopericarditis

Abstract

Acute myocarditis usually develops after viral infections and is often asymptomatic. However, pericarditis can accompany it. Myopericarditis due to Epstein-Barr virus (EBV) infection is very rare. A 16-year-old male presented with severe chest pain. Cardiac enzymes were elevated and electrocardiographic examination revealed widespread decreased voltage and ST-T changes on the DII, DIII, aVF, V5, and V6 leads. Left ventricular systolic functions were normal and pericardial effusion was present on echocardiographic evaluation. EBV-VCA IgM and EBV-PCR were positive; EBV myopericarditis was diagnosed in the patient. The patient responded well to intravenous immunoglobulin (IVIG) and nonsteroidal antiinflammatory therapy and recovered completely. Myopericarditis should be considered in the differential diagnosis while evaluating patients with chest pain. The Epstein-Barr virus should be kept in mind even though it rarely causes myopericarditis

Articles accepted for publication in the Turkish Journal of Pediatrics are put in the order of publication, with at least 10 original articles in each issue, taking into account the acceptance dates. If the articles sent to the reviewers for evaluation are assessed as a senior for publication by the reviewers, the section editor and the editor considering all aspects (originality, high scientific quality and citation potential), it receives publication priority in addition to the articles assigned for the next issue.


The aim of the Turkish Journal of Pediatrics is to publish high-quality original research articles that will contribute to the international literature in the field of general pediatric health and diseases and its sub-branches.
