Fiziksel Tıp ve Rehabilitasyon Polikliniğine Kas-iskelet Sistemi Ağrısı Şikayeti ile Başvuran 5 Yaş ve Üstü Çocukların Değerlendirilmesi

Abstract

Amaç: Kas-iskelet sistemi ağrıları pek çok ülkede çocukluk çağında sağlık kuruluşlarına en sık başvuru nedenlerinden biridir. Kas-iskelet sistemi ağrı sebeplerinin ve sıklıklarının bulunması sağlık hizmetlerinin planlanmasında ve ulusal rehberler geliştirilmesinde önemlidir. Bu çalışmada fiziksel tıp ve rehabilitasyon polikliniğine kas-iskelet sistemi ağrısı ile başvuran 5 yaş ve üzeri çocuklarda kas-iskelet sistemi ağrılarının sebeplerinin ve sıklıklarının bulunması amaçlanmıştır.Gereç ve Yöntemler: Bu çalışmada 1 Kasım 2014 ile 1 Mayıs 2015 tarihleri arasında, Fiziksel Tıp ve Rehabilitasyon polikliniğine kas-iskelet sisteminde ağrı şikayeti ile ardı sıra başvuran 5 yaş ve üzeri çocuk hastaların kayıtları geriye dönük olarak incelendi. Hastaların demografik ve klinik özellikleri kaydedildi. Kas-iskelet sistemi ağrı bölgeleri ve etiyolojileri sınıflandırıldı.Bulgular: Çalışmaya dahil edilen 147 çocuk hastanın ortanca yaşı 14 (11-16) [ ortanca (%25-%75 aralık)] olup, %57,8’i erkek ve %14,3’ü obezdi. Hastaların %32’sinde kronik kas-iskelet sistemi ağrısı olduğu bulundu. Başvuru şikayetleri sırasıyla bel ağrısı (%21,1), yumuşak doku kaynaklı ağrı (%21,1), diz ağrısı (%16,3), boyun-sırt ağrısı (%14,3), diğer eklem ağrıları (%12,9), ayak bileği ağrısı (%5,4), kemik ağrısı (%2,4), kalça ağrısı (%1,4), nöropatik ağrı (%1,4) şeklinde idi. En sık kas-iskelet sistemi ağrı etiyolojileri travma (%35,4) ve mekanik, aşırı kullanımdı (%32,6). Çalışmaya alınan hastalar 5-9 yaş ve 10-18 yaş arasında olanlar olarak iki gruba ayrıldı. 5-9 yaş arası grupta büyüme ağrısı (%20), osteokondrozlar (%8,3); 10-18 yaş arası grupta ise enflamatuar artritler (%10,9), osteokondrozlar (%8,9), hipermobilite sendromu (%5,7), non-spesifik ağrı (%4,9), viral enfeksiyonlara bağlı kas eklem ağrıları (%2,4) diğer kas-iskelet sistemi ağrısı etiyolojileri olarak bulundu.Sonuç: Çocuk hasta grubunda kas-iskelet sistemi ağrısına yol açan en sık iki etiyoloji travma ve mekanik, aşırı kullanımdır. Bu iki durumu takip eden diğer etiyolojiler yaşa göre değişkenlik göstermektedir. Ülkemizde çocuklarda kas-iskelet sistemi ağrısı ile ilgili yapılacak çok merkezli ve prospektif, takipli çalışmalara ihtiyaç vardır.

Abstract

Objective: Musculoskeletal pain is one of the most common complaints in childhood leading to presentation to health institutions in many countries. Determining the prevalence and causes of musculoskeletal pain are important for the planning of health services and the development of national guidelines. This study aimed to find the prevalence and causes of musculoskeletal pain in children aged 5 years and older presenting to the Physical Medicine and Rehabilitation clinic because of musculoskeletal pain.Material and Methods: In this study, the records of children aged 5 years and older consecutively presenting to the Physical Medicine and Rehabilitation outpatient clinic between November 2014 and May 2015 were retrospectively analyzed. Demographic and clinical characteristics of the patients were recorded. The regions and etiologies of musculoskeletal pain were classified.Results: The study included 147 children with a median age of 14 years [median (range 25% -75%)]; 57.8% were male and 14.3% were obese. The editorial board will invite an external and independent editor to manage the evaluation processes of manuscripts submitted by editors or by the editorial board members of the journal. The Editor in Chief is the final authority in the decision-making process for all submissions. Articles accepted for publication in the Turkish Journal of Pediatrics are put in the order of publication, with at least 10 original articles in each issue, taking into account the acceptance dates. If the articles sent to the reviewers for evaluation are assessed as a senior for publication by the reviewers, the section editor and the editor considering all aspects (originality, high scientific quality and citation potential), it receives publication priority in addition to the articles assigned for the next issue.


The aim of the Turkish Journal of Pediatrics is to publish high-quality original research articles that will contribute to the international literature in the field of general pediatric health and diseases and its sub-branches.
