Miyoperikardit ile Komplike Olan Bir Bakteriyel Pnömoni Olgusu

Abstract

Pnömoniye eşlik eden bir miyoperikardit olgusu sunulmaktadır. On yedi yaşındaki erkek hasta göğüs ağrısı yakınması ile başvurduğu acil servisten akut miyokart enfarktüsü ön tanısı ile hastanemize yönlendirilmişti. Hastanın başvurusunda EKG’de ST elevasyonu (DII, DIII, aVF, V3-V6), yüksek kardiyak troponin I (15.4 ng/ml, normal değer <0.06) ve akciğer grafisinde infiltrasyon mevcuttu. Pnömoni ve miyoperikardit tanısıyla yatırılarak antibiyotik ve anti-inflamatuvar tedavi uygulandı. Tedavisinin 10. gününde taburcu edildi. Miyoperikardit’in en sık nedeni viral enfeksiyonlardır; ancak nadir de olsa bakteriyel enfeksiyonlar sırasında da miyoperikardit görülebilir. Sıklıkla subkliniktir ve kardiyak belirti ve bulgular altta yatan sistemik enfeksiyon ve inflamasyon tarafından gölgelenebilir. Tedavide istirahat, fiziksel aktiviteden uzak durma, anti-inflamatuvar tedavi ve mümkünse altta yatan hastalığın tedavisi uygulanır.

A Case of Bactreial Pneumonia Complicated by Myopericarditis

Abstract

We report a case of myopericarditis concomitant with pneumonia. The patient presented at the emergency service with chest pain and had been referred to our hospital with a diagnosis of myocardial infarction. At the time of admission he had ST elevation (DII, DIII, aVF, V3-V6) on ECG, cardiac troponin I elevation (15.4 ng/ml, N:<0.06) and pneumonic infiltration on chest radiography. He was hospitalized, and antibiotic and anti-inflammatory treatment was administered. He was discharged after 10 days of treatment. Although the most common causative agent of myopericarditis is viruses, bacterial infections can also rarely lead to myocarditis. Most cases of myopericarditis are subclinical and the cardiac symptoms and signs can be masked by systemic manifestations of infection and inflammation. Management of myopericarditis includes avoidance of physical activity, administering anti-inflammatory treatment and possibly treatment of the underlying disease

If the articles sent to the reviewers for evaluation are assessed as a senior for publication by the reviewers, the section editor and the editor considering all aspects (originality, high scientific quality and citation potential), it receives publication priority in addition to the articles assigned for the next issue.
