Çocuklarda Ürtikeryal Vaskülit

Abstract

Giriş: Ürtikeryal vaskülit histolojik olarak vaskülit bulguları gösteren, klinik olarak persistan ürtikeryal döküntülerle karakterize bir klinikopatolojik durumdur. Patogenezde tip 3 hipersensitivite reaksiyonunun ve dolaşımdaki immün komplekslerin rol oynadığına yönelik bulgular mevcuttur. Hastalık sıklıkla orta yaşlı kadınlarda bildirilmiştir. Literatürde çocuk hastaları içeren az sayıda yayın mevcuttur.Olgular: Burada ürtikeryal vaskülitli 4 çocuk hasta bildirilmiştir. Hastalar 9 aylık ve 2 yaşında olan 2 kız, 10 ve 2 yaşında 2 erkekti. Hastaların 2 sinde 5 gün, birinde ise 4 gün ve birinde is 7 gün devam eden ürtiker plakları gözlendi ve lezyonlar yer yer purpura ve ekimoza dönüşüp hiperpigmentasyon bırakarak iyileşti. Hastalardan birine cilt biyopsisi yapıldı. Hastaların laboratuvar incelemelerinde tam kan sayımı, akut faz reaktanları, serum kompleman 3 ve 4, immunglobülinler, ASO, AST, ALT ve idrar tetkiki yapıldı. İki hastada sedimentasyon hızı yüksekti. Dört hastanın da serum kompleman düzeyleri normal bulundu.Sonuç: Hastaların klinik ve laboratuvar bulguları literatür bilgileri ile karşılaştırılarak, çocukluk çağında nadir görülen ürtikeryal vaskülit ve ayırıcı tanısı tartışılmıştır.

URTICARIAL VASCULITIS IN CHILDHOOD

Abstract

Introductron:Urticarial vasculitis is a clinicopathological condition characterized by persistent urticarial weals which show vasculitic features on histology. There is evidence that type III hypersensitivity reaction and circulating immune complexes play role in the pathogenesis. The majority of patients are middle aged women. There are limited number of previously reported pediatric cases.Cases: We herein report four pediatric cases of urticarial vasculitis. The patients were two ( 9 month old and a 2 year old) females and two ( 2 year old and a 10 year old ) males. Urticarial plaques persisted for 4-7 days the patients. The lesions became purpuric and echymotic and healed by leaving hyperpigmentation. Skin biopsy was performed in one patient. In laboratory examination complete blood count, acute phase reactants, serum complement levels, immunoglobulins, ASO titers, AST, ALT levels and urinalyis were performed. Two patients had high sedimentation rate and all four had normal serum complement levels. Other laboratory findings were unremarkable.Conclusion: Clinical anad laboratory findings of patients have been compared by the literature data. Manuscripts submitted to the Turkish Journal of Pediatric Disease will go through a double-blind peer-review process. Each submission will be reviewed by at least two external, independent peer reviewers who are experts in the field, in order to ensure an unbiased evaluation process. The editorial board will invite an external and independent editor to manage the evaluation processes of manuscripts submitted by editors or by the editorial board members of the journal. The Editor in Chief is the final authority in the decision-making process for all submissions. Articles accepted for publication in the Turkish Journal of Pediatrics are put in the order of publication, with at least 10 original articles in each issue, taking into account the acceptance dates. If the articles sent to the reviewers for evaluation are assessed as a senior for publication by the reviewers, the section editor and the editor considering all aspects (originality, high scientific quality and citation potential), it receives publication priority in addition to the articles assigned for the next issue.


The aim of the Turkish Journal of Pediatrics is to publish high-quality original research articles that will contribute to the international literature in the field of general pediatric health and diseases and its sub-branches. It also publishes editorial opinions, letters to the editor, reviews, case reports, book reviews, comments on previously published articles, meeting and conference proceedings, announcements, and biography.
