Çocukluk Çağı Graves Hastalığında Radyoaktif İyot Tedavisi

Abstract

Graves hastalığında radyoaktif iyot tedavisi (RIT) oldukça etkin, ucuz, kolay uygulanabilen ve güvenilir bir yöntem olmakla birlikte, çocukluk çağındaki kullanımı yaygın değildir. Bu çalışmada antitiroid ilaç tedavisine yanıt vermeyen 7 Graves olgusunda uygulanan RIT sunulacaktır. Çalışmaya 11-14 yaş arasında, 6' sı kız biri erkek 7 Graves olgusu alındı. Tüm olgulara ilk seçenek olarak antitiroid ilaç tedavisi (propiltiourasil 3-6 mg/kg/gün) başlandı. Yaklaşık 2-6 yıl tedavisi sürdürülen 6 olguda iyileşme olmaması, bir hastada ise ilaca bağlı hepatotoksisite gelişmesi nedeni ile RIT 10-15 mCi dozunda uygulandı. Tüm olgularda 3 ay içinde remisyon sağlanırken bir yıllık izlemlerinde relaps veya RIT' e bağlı herhangi bir komplikasyon görülmedi. Bu çalışma ile antitiroid ilaç tedavisine yanıt alınamayan veya ciddi yan etkisi görülen Graves hastalığında, çocuk ve adolesanlarda da RIT' nin etkin bir tedavi olduğu sonucuna varıldı.

RADIOIODINE TREATMENT OF GRAVES DISEASE IN CHILDHOOD

Abstract

Radioiodine therapy (RIT) is a safe and effective treatment option for management of childhood-onset Graves' disease. The aim of this study was to evaluate the efficacy of RIT in 7 childhood-onset Graves' patients who had relapsed following the initial medical therapy. The patients consisted of 6 females and 1 male whose ages ranged from 11 to 14 years. All patients received propylthiouracil (3-6 mg/kg/day) as the initial antithyroid drug therapy. During 2-6 years follow-up, 6 patients did not show remission and the remaining one patient developed toxic hepatitis due to the antithyroid drugs. RIT (10-15 mCi) were given following antithyroid drug therapy. Remission was achieved in all patients within 3 months. No complication related to RIT was observed during 1 year follow-up. Our data demonstrated that RIT is an effective and safe therapy for childhood-onset Graves' patients who has relapsed disease after initial medical therapy

Articles accepted for publication in the Turkish Journal of Pediatrics are put in the order of publication, with at least 10 original articles in each issue, taking into account the acceptance dates. If the articles sent to the reviewers for evaluation are assessed as a senior for publication by the reviewers, the section editor and the editor considering all aspects (originality, high scientific quality and citation potential), it receives publication priority in addition to the articles assigned for the next issue.
