Son yıllarda keşfedilen leptin proteininin insan fizyolojisinde besin alımı, obezite, enerji dengesinin düzenlenmesi, puberte başlangıcı ve kontrolü, hipotalamo-hipofizer fonksiyonların düzenlenmesi gibi önemli roller oynadığı bilinmektedir. Leptinin Tip-1 diyabet ve insülin ile ilişkisi ise tartışmalıdır. Bu çalışmada diyabetik ketoasidoz (DKA) ile başvuran Tip-1 diyabetli hastalarda insülin tedavisinin serum leptin düzeylerine etkisinin araştırılması amaçlandı. Mayıs 2003-Mayıs 2004 tarihleri arasında kliniğimize diyabetik ketoasidoz (DKA) ile başvuran ve ilk kez tanı alan 22 (12 kız, 10 erkek ort. yaş, 7.8±3.9) hasta çalışma grubunu oluşturdu. Birinci kontrol grubu Tip-1 diyabet tanılı komplikasyonsuz takip edilen 22 hastadan ve 2. kontrol grubu benzer yaştaki sağlıklı çocuklardan oluştu. Yeni tanı alan hastalarda insülin tedavisi başlanmadan ve başlandıktan 24 saat sonra ve takipli Tip-1 diyabetes mellitus tanılı hastalarda sabah insülin dozundan önce olmak üzere tüm gruplarda leptin düzeyi bakılarak karşılaştırıldı. Tedavi öncesinde DKA grubunda leptin düzeyleri kontrol gruplarına göre düşük bulunmakla birlikte istatistiksel olarak anlamlı fark yoktu sırasıyla; (4.52±2.7, 7.11±5.74, 6.99±5.25 ng/ml p>0.05). Takipli Tip-1 DM grubunda ortalama leptin düzeyi sağlıklı gruba göre yüksek idi. Çalışma grubunda tedavi başlandıktan sonra leptin düzeyinde artış oldukça anlamlı bulundu (z:-3.84, p:0.000). Diyabetik Ketoasidozlu (DKA' lu) hastalarda başta insülin eksikliği olmak üzere ketoasidozun indirekt etkisi leptin düzeyinde azalmaya neden olabilir. İnsülin tedavisi ile leptin düzeyinin artması insülinin leptin yapımını arttırmasına bağlanabilir. Ayrıca takipli Tip-1 DM grubunda ortalama leptin düzeyinin sağlıklı gruptan yüksek olması insülinin leptin üzerine uyarıcı etkisini desteklemektedir.
Leptin has important roles in food intake, obesity, regulation of energy balance, puberty onset and regulation of hypothalamo-pituitary functions. The relation between leptin and type-1 diabetes is controversial. In this study we aimed to evaluate the effect of the insulin therapy on the serum levels of leptin in the children with new-onset diabetes mellitus (DM) presented in diabetic ketoacidosis (DKA) .The study included 22 new-onset Type 1 DM patients admitted to the Bakırkoy Maternity and Children's Hospital, Pediatric Endocrinology unit presented with DKA (12 girls, 10 boys aged 7.8±3.9) between May 2003 and May 2004. As the first control group 22 Type 1 DM patients followed up without any complications and as the second control group 22 healthy children with comparable age and gender served. Serum leptin was measured before and 24 hours after the insulin treatment in new-onset DM patients and in the morning before the insulin treatment in the control groups. Serum levels of leptin in DKA patients before the treatment were lower than those of the control groups (4.52±2.7 vs. 7.11 ±5.74 vs. 6.99 ±5.25 ng/ml p> 0.05), statistically nonsignificant. Leptin was found significantly increased after the insulin therapy in the patients with DKA ( p:0.000). Insulin deficiency and probably indirect effect of ketoacidosis cause decrease in serum leptin levels in DKA patients. The increase in the leptin levels with the insulin treatment may be explained by the fact that insulin increases the leptin production. The higher leptin levels in type 1 DM patients than those in the healthy children suggests that insulin has a stimalating effect on the leptin production

Manuscripts submitted to the Turkish Journal of Pediatric Disease will go through a double-blind peer-review process. Each submission will be reviewed by at least two external, independent peer reviewers who are experts in the field, in order to ensure an unbiased evaluation process. The editorial board will invite an external and independent editor to manage the evaluation processes of manuscripts submitted by editors or by the editorial board members of the journal. The Editor in Chief is the final authority in the decision-making process for all submissions. Articles accepted for publication in the Turkish Journal of Pediatrics are put in the order of publication taking into account the acceptance dates. If the articles sent to the reviewers for evaluation are assessed as a senior for publication by the reviewers, the section editor and the editor considering all aspects (originality, high scientific quality and citation potential), it receives publication priority in addition to the articles assigned for the next issue.
The aim of the Turkish Journal of Pediatrics is to publish high-quality original research articles that will contribute to the international literature in the field of general pediatric health and diseases and its sub-branches. It also publishes editorial opinions, letters to the editor, reviews, case reports, book reviews, comments on previously published articles, meeting and conference proceedings, announcements, and biography. In addition to the field of child health and diseases, the journal also includes articles prepared in fields such as surgery, dentistry, public health, nutrition and dietetics, social services, human genetics, basic sciences, psychology, psychiatry, educational sciences, sociology and nursing, provided that they are related to this field. can be published.
