How Do Medical Graduates Evaluate Their Clinical Performance with Undergraduate Medical Education?: A Qualitative Study

Abstract

Aim: It is recommended to develop undergraduate medical education by taking the performances of the graduates or the opinions of the graduates, as well as the evaluations in the undergraduate medical education process, whether the medical graduates have achieved the targeted competencies or not. The aim of the study is to reveal the performance of Karadeniz Technic University Faculty of Medicine graduates and the factors that affect them, and the views of graduates on the relationship between these factors and the dynamics of the education received before graduation. Methods: This is a qualitative designed study. In order to determine the participants, graduates who were assigned to KTU in the 100th draw of the Ministry of Health's Compulsory Sercives and who started to work were included in the study by easily accessible sampling method. Communication with alumni was established through the contact information obtained from the alumni office of KTU Faculty of Medicine. Inclusion criterias were; not having received any residency training, having worked at the assigned service for at least 6 months, having a maximum of 2 years of employment history, having passed at most 2 years after graduation. In-depth interviews were conducted online via the Zoom platform. The data were prepared with expert opinion and revealed through semi-structured questions. Creswell's analysis approach of qualitative data analysis was adopted. Ethics committee approval was obtained for the study. Results: Within the scope of the study, 55 medical graduates were determined. Two separate focus group meetings conducted on the Zoom platform were held with the participation of 16 people in total. Three categories emerged as physician performance, quality of education, education and field conflicts. The theme at the center of all the analyzed data was whether the graduates felt themselves as "competent physician". It has been revealed that the factors that support and limit being a competent physician are also affected by the implicit learning climate. Conclusions: In the study, it was revealed that graduates have difficulties in communication and process management, especially in complex situations, and that this is the result of poorly structured clinical education. The professional inadequacies felt can be overcome with an outcome-based education approach designed in line with the targeted competencies. It has been revealed that the graduates take responsibility among the trainings they receive during their student years, they can communicate well with their trainers, they adopt and support the educational activities that they think will directly affect the medical performance they receive feedback from their physicians. It is recommended to develop policies that increase learner and trainer motivation. The method used in the study can be easily applied by institutions providing medical education. In this way, it will be possible to reveal the valuable views of the graduates about the medical education offered in the provision of health services, with a method with few confounding factors. It was revealed that the participants did not feel fully ready for the provision of health services and they presented their opinions with the focus of being a 'competent physician'.

Tıp Mezunları Hekimlik Performanslarını Mezuniyet Öncesi Tıp Eğitimiyle Nasıl İlişkilendiriyor?: Kalitatif Bir Çalışma

Abstract

Amaç: Tıp mezunlarının hedeflenen yetkinliklere ulaşıp ulaşmadıklarını, mezuniyet öncesi süreçteki değerlendirmelerin yanında, mezunların saha performansları veya mezun görüşlerinin de alınarak değerlendirilmesi ve mezuniyet öncesi eğitimin geliştirilmesi önerilmektedir. Çalışmanın amacı Karadeniz Teknik Üniversitesi (KTÜ) Tıp Fakültesi mezunlarının sahadaki performanslarını ve etkileyen faktörleri, bu faktörlerle mezuniyet öncesinde alınan eğitimin dinamikleri arasındaki ilişkilerine yönelik mezun görüşlerini ortaya çıkarmaktır. Yöntem: Çalışma kalitatif desende tasarlanmıştır. Katılımcıların belirlenmesi için, Sağlık Bakanlığı’nın Devlet Hizmeti Yükümlülüğü 100. kurasında KTÜ’ye atanan ve göreve başlayan mezunlardan kolay ulaşılabilir örneklem yöntemi ile mümkün olan en farklı çalışma alanlarında hizmet eden mezunlar çalışmaya dahil edilmiştir. Mezunlar ile iletişim KTÜ Tıp Fakültesi’nin mezunlar ofisinden elde edilen iletişim bilgileri aracılığıyla kurulmuştur. Araştırmaya dahil etme kriterlerimiz; herhangi bir asistanlık eğitimi almamış olmak, en an 6 ay atandığı yerde bir fiil çalışmış olmak, en fazla 2 yıl görev geçmişi olması, mezuniyetinin üzerinden en fazla 2 yıl geçmiş olmak, şeklinde belirlenmiştir. Çalışmaya gönüllü olarak kabul eden mezunlarla derinlemesine görüşmeler Zoom platformu üzerinden çevrim içi yapılmıştır. Veriler uzman görüşü ile hazırlanmış, yarı yapılandırılmış sorular üzerinden ortaya çıkarılmıştır. Nitel veri analizlerin Creswell’in analiz yaklaşımı benimsenmiştir. Çalışma için etik kurul onayı alınmıştır. Bulgular: Çalışma kapsamında 55 tıp mezunu belirlenmiştir. Zoom platformu üzerinden yürütülen iki ayrı odak grup görüşmesi toplamda 16 kişinin katılımı ile gerçekleştirilmiştir. Verilerin analizi sonucunda hekimlik performansı, eğitimin niteliği, eğitim ve saha uyuşmazlıkları adlı 3 kategori ortaya çıkmıştır. Analizi yapılan tüm verilerin odağındaki tema mezunların kendilerini ‘‘yetkin hekim’’ hissedip hissetmemesi yer almaktaydı. Yetkin bir hekim olmayı destekleyen ve sınırlayan faktörlerin örtük öğrenme ikliminden de etkilendiği ortaya çıkmıştır. Sonuç: Çalışmada mezunların özellikle karmaşık durumlarda iletişim ve süreç yönetiminde zorlandıkları, bunun ise iyi yapılandırılmamış klinik eğitimin sonucu olduğu görüşleri ortaya çıkmıştır. Hissedilen mesleki yetersizlikler, hedeflenen yetkinlikler doğrultusunda tasarlanmış çıktı odaklı eğitim yaklaşımı ile aşılabilir. Mezunların öğrencilik yıllarında aldıkları eğitimler arasında sorumluluk aldıkları, eğiticisi ile iyi iletişim kurabildiği, eğiticisinden geribildirim aldığı hekimlik performansını doğrudan etkileyeceğini düşündüğü eğitim faaliyetlerini daha çok benimsediği, desteklediği ortaya çıkmıştır. Öğrenen ve eğitici motivasyonunu artırıcı politikaların geliştirilmesi önerilmektedir. Çalışmada kullanılan yöntem, tıp eğitimi veren kurumlar tarafından rahatlıkla uygulayabilir. Böylelikle, sunulan tıp eğitimine yönelik mezunların kıymetli görüşlerini sağlık hizmet sunumunda elde ettikleri, karıştırıcı faktörlerin az olduğu bir yöntemle ortaya çıkarmak mümkün olacaktır. Katılımcıların kendilerini sağlık hizmeti sunumuna tam olarak hazır hissetmedikleri ve görüşlerini ‘yetkin hekim’ olma odağında sundukları ortaya çıkmıştır.
