Medicolegal importance of toxicological pathologies of the central nervous system

Abstract

Introduction: Toxicological diseases of the central nervous system display various morphological and clinical findings. Within this scope, deaths due to CO intoxications are seen with considerable frequency in our country and in the world, as well. The aim of our study is to review the causes, morphological features, and the clinical outcomes of these morphological changes of the most frequent pathologies of the central nervous system. Results: CO and methanol intoxications are the most considered toxicological diseases of the central nervous system. Furthermore, ethylene glycole, lead, arsenic, mercury, and talium intoxications lead to clinical and morphological changes of the central and peripheric nervous system, as well. Conclusion: It is well known that various neuro-physiological changes occur in the central nervous system of survivers of toxications. Therefore, in legal autopsy applications, even in traumatic deaths, considering the intoxication stories of the cases, detailed macroscopical and microscopical assessment of the brain remains very important

Merkez sinir sisteminde toksik nedenli patolojilerin adli tıp açısından önemi

Öz

Giriş ve amaç: Merkezi sinir sisteminin toksik hastalıkları, oldukça belirgin morfolojik ve klinik bulgularla kendini gösterir. Bunlar içerisinde, özellikle CO zehirlenmelerine bağlı ölümler, ülkemizde ve dünyada önemli sıklıkta görülmektedir. Amacımız toksik nedenlere bağlı en sık karşılaşılan santral sinir sistemi patolojilerinin nedenlerini, morfolojik özelliklerini ve bu morfolojik değişimlerin yol açtığı klinik sonuçları gözden geçirmek ve adli tıp açısından önemini vurgulamaktır. Bulgular: Santral sinir sisteminin toksik hastalıkları içerisinde, üzerinde en çok durulanlar CO ve metanol zehirlenmeleridir. Ayrıca, etilen glikol, kurşun, arsenik, civa ve talyum zehirlenmeleri sonucunda da santral ve periferik sinir sistemini ilgilendiren klinik ve morfolojik değişimler oluşmaktadır. Sonuç: Zehirlenmeden sonra yaşayan olgularda santral sinir sisteminde farklı nörofizyolojik değişimler olur. Bu nedenle, adli otopsi pratiğinde travmaya bağlı gelişen ölümler içinde olmak üzere, olgunun zehirlenme öyküsü gözönüne alınarak, beynin ayrıntılı patolojik makroskopik ve mikroskopik değerlendirilmesi önem taşımaktadır
