Relationship between tumour size and histological parameters of low grade (benign) phyllodes tumours

Abstract

Introduction and aim: Phyllodes tumour is a biphasic tumour of breast which is characterized by benign epithelial elements and cellular spindle-celled stroma. To explain different behaviour modalities of benign phyllodes tumours, we investigated the relationship of stromal and epithelial features with each other and tumour size. Material and method: There were 29 tumors from 27 patients diagnosed as benign phyllodes tumor at SSK Tepecik Teaching Hospital Pathology Department between 1995-2003. They are divided into three groups according to tumor size. Statistical comparison between three groups and among each group was made regarding stromal cellularity, stromal pleomorphism, stromal hyalinization, stromal edema, stromal vessel density and epithelial cellularity. Results: None of the histological parameters showed statistically significant differences according to tumor size. Regardless of tumor size, in benign phyllodes tumors, a positive correlation between stromal cellularity and stromal pleomorphism and a negative correlation between stromal cellularity and stromal vessel density was found (p=0.005 and p=0.028 respectively). Conclusion: Proportional increase in stromal cellularity and stromal pleomorphism and seemingly disproportional decrease in stromal vessel density may take place in predictive parameters

Düşük dereceli fillodes tümörlerde tümör boyutu ile histolojik parametrelerin ilişkisi

Öz

Giriş ve amaç: Fillodes tümör, benign epiteliyal elemanlar ve sellüler iğsi hücreli stroma ile belirli memenin bifazik tümörüdür. Benign fillodes tümörlerde farklı biyolojik davranış modellerine ışık tutabilmek amacıyla, stromal ve epiteliyal özelliklerin tümör çapı ve birbirleriyle ilişkisine yönelik bir çalışma planlandı. Gereç ve yöntem: SSK Tepecik Eğitim Hastanesi Patoloji Laboratuarı’nda 1995-2003 yılları arasında, 27 olguya ait 29 tümör benign fillodes tümör tanısı aldı. Tümörler, çaplarına göre 3 gruba ayrılarak, stromal sellülarite, stromal pleomorfizm, stromal hiyalinizasyon, stromal ödem ve stromal vasküler yoğunluk ile epiteliyal sellülarite açısından gruplar arası ve grup içi istatistiksel karşılaştırma yapıldı. Bulgular: Hiçbir histolojik parametre için, gruplar arasında istatistiksel olarak anlamlı farklılık olmadığı, yani hiçbir parametrenin tümör çapı ile ilişkili olmadığı görüldü. Tümör çapından bağımsız olarak ise, stromal sellülarite ile stromal pleomorfizm arasında anlamlı pozitif, stromal sellülarite ile stromal vasküler yoğunluk arasında anlamlı negatif korelasyon vardı (sırası ile p=0.005, p=0.028). Sonuç: Benign fillodes tümörlerde birbirine paralel artış gösteren stromal sellülarite ve stromal plemorfizm ile, bu iki parametre ile ters orantılı görünen stromal vasküler yoğunluk belki de gelecekte birer prediktif parametre olarak kullanılabilir. Bu konuda daha geniş serili çalışmalara ihtiyaç vardır
