Romatoid artritli ve osteoartritli kadınlarda görülen ayak deformitelerinin değerlendirilmesi

Abstract

Amaç: Bu çalışmanın amacı, romatoid artrit (RA) ve osteoartrit (OA) tanısıyla izlenmekte olan 40 yaş ve üzeri kadın hastalarda görülen ayak-ayak bileği problemlerini belirlemek, farklılıkları araştırmak ve aynı yaş grubu sağlıklı kadınların ayaklarıyla karşılaştırmaktı. Gereç ve yöntem: Çalışmaya toplam 67 kadın olgu alındı. Olguların 26’sı RA, 21’i OA ve 20’si sağlıklı bireylerden oluşmaktaydı. Olguların ayağa ilişkin yakınma ve semptomları, eklem hareket genişlikleri, mevcut deformitelerin şiddetleri, ayak fonksiyon indeksi ve ağrıları değerlendirildi. Sonuçlar: Eritem, sabah sertliği gibi inflamatuar belirtilerin RA’lı hasta grubunda daha fazla olduğu, ödem, metatarsofalangeal hassasiyet, metatarsofalangeal ve proksimal interfalangeal eklem nasırlarının her üç grupta da değişen oranlarda ortaya çıktığı saptandı. Metatarsofalangeal eklem ağrısının her üç grupta fazla olduğu ve RA’da aktiviteyle ağrı artışının belirgin olduğu bulundu. Ayak Fonksiyon İndeksi skorlarının ve toplam eklem limitasyonlarının RA’lı grupta daha fazla olduğu saptandı (p0.05). Tartışma: Araştırmamız RA’lı hastalardaki dalgalanmalı inflamatuar sürecin fonksiyonu etkileyen ağrı ve limitasyon oluşumuyla ayak mekaniğini olumsuz şekilde etkilediğini, OA’lı hastalardaki artiküler mekanik süreçlerin ayak yapılarını zorlamakla birlikte daha az semptomla birlikte görüldüğünü ortaya koydu. Ayrıca orta yaş üzeri sağlıklı kadın olgularda da ayakta ortaya çıkan dejeneratif değişikliklerin göz ardı edilemeyecek düzeyde olduğunu gösterdi
