PES PLANUSUN ALT EKSTREMİTE BİYOMEKANİĞİNE VE FONKSİYONELLİĞE ETKİSİ

Abstract

Amaç:Çalışmanın amacı, pes
planusu olan ve olmayan genç sedanter bireyler arasındaki farkın ayağın ve alt
ekstremitenin biyomekanik özellikleri, ayak fonksiyonu ve bel ağrısı
açılarından incelenmesidir.

Yöntem::
Çalışma 30 kadın, 30 erkek olmak üzere 60 sağlıklı sedanter bireyin katılımı
ile gerçekleştirildi. Tüm bireyler bir kez navikülar yükseklik, ayak
fonksiyonu, alt ekstremite kas kısalıkları, kas kuvvetleri, Q açısı, pelvik
inklinasyon ve bel ağrısı açılarından değerlendirildi. Bireylerin dominant ve
dominant olmayan ayakları Navikülar Düşme Testi sonuçlarına göre normal ya da
düşük medial longitudinal ark (MLA) yüksekliğine sahip olmak üzere iki gruba
ayrıldı.

ormal ve düşük MLA’ya sahip olan ayakların
oluşturduğu iki grup karşılaştırıldı.

Sonuçlar:MLA’sı düşük
olanlarda normal olanlara göre dominant tarafta soleus ve gastroknemius kas
kısalığı ile pelvik inklinasyonun, dominant olmayan tarafta hamstring kas
kısalığının daha fazla olduğu bulundu (p=0,001, p=0,027, p=0,038, p=0,004). MLA’sı
düşük ve normal olanlar arasında tibialis posterior ve peroneal kas kuvveti, Q
açısı ile Oswestry, AOFAS ve VAS FA skorlarında anlamlı bir fark olmadığı
bulundu (p>0,05). Dominant olmayan tarafta Q açısı ve pelvik inklinasyon
arasında düşük düzeyde ancak anlamlı bir negatif korelasyon olduğu bulundu
(r=-0,345, p=0,007). Pelvik inklinasyon ile Oswestry skoru arasında anlamlı bir
ilişki olmadığı bulundu (p>0,05).

Tartışma:Sonuçlarımız, genç
sedanter bireylerde pes planusun alt ekstremite biyomekaniklerini
etkileyebildiğini göstermekle birlikte bel ve ayak ağrısı parametreleri ve kas
kuvvetinin bu durumdan etkilenmediğini ortaya koymuştur.
