Vorikonazol ile tedavi edilen bir Aspergillus flavus kompleks keratit olgusu

Abstract

Fungal keratitlere, Candida spp. cinsi mayalar ile Fusarium spp. Aspergillus spp. ve Alternaria spp. gibi çeşitli filamentöz mantarlar etken olabilir. Fungal keratitlerde epidemiyolojinin ve predispozan risk faktörlerinin bilinmesi ve doğru laboratuvar tanı ile hastaya etkin antifungal tedavinin başlanması uygun yaklaşımdır. Fungal keratitleri tedavi etmek zordur ve intraoküler tutulum gibi ciddi bir risk faktörü taşır. Geleneksel antifungal ilaçlar genellikle iyi topikal transkorneal penetrasyon sağlar, ancak sistemik tedavi ile göz içi penetrasyonda sınırlı kalırlar. Bu yazıda, oral ve topikal vorikonazol ile başarılı şekilde tedavi edilen A. flavus kompleks keratit olgusu sunulmuştur. Fungal keratitler nadir gözlense de halen kötü prognozla seyretmektedirler. Fungal keratitlerde tanının doğru ve hızlı gerçekleştirilmesi ve tedavi seçenekleri hakkında doğru bilgiye sahip olunması önem arz eder

A case of Aspergillus flavus complex keratitis, treated with voriconazole

Abstract

Fungal keratitis can be caused by yeasts Candida spp. and various filamentous fungi such as Fusarium spp. Aspergillus spp. and Alternaria spp. In fungal keratitis it would be on appropriate approach to know risk factors of epidemiology and predisposing risks, and to start on effective antifungal treatment to the patient with proper laboratory diagnosis. Fungal keratitis is difficult to treat and carries a serious risk factor like intraocular involvement. Traditional antifungal drugs usually have good topical transcorneal penetration, but they are limited to intraocular penetration and systemic therapy. In this paper, it is presented a case of A. flavus complex keratitis successfully treated with oral and topical voriconazole. Although fungal keratitis is occured rarely, it still has a serious prognosis. In fungal keratitis it is significant to perform accurate and rapid diagnosis and have accurate knowledge about treatment options
