Determining The Problems and Quality of Life of Patients Receiving The Treatment of Interventional Hemorrhoid

Abstract

Aim: This study was conducted as a descriptive study in order to determine the problems experienced by patients undergoing interventional hemorrhoids treatment and their quality of life, and it is thought that as a result of the study, it will guide the practice of nursing care to prevent problems that may occur after interventional hemorrhoid treatment and affect the quality of life of patients less, and contribute to the literature.
Method: 73 patients who establish and agree to participate in study and to whom interventional hemorrhoids treatment applied at a Health Research and Application Center General Surgery and proctology unit of an university in Ankara were considered as study sample. The sociodemographic questionnaire, post-treatment questionnaire, questionnaire designed to determine complications, and Roll Royce Quality of Life Scale were used to collect the data. In the analysis of the data obtained from the study; The number, percentage was evaluated using the “Paired Sample-t” test (t-table value) for comparisons of two normally distributed dependent groups and the “Wilcoxon” test (Z-table value) for comparisons of two non-normally distributed dependent groups.
Result: It was determined that the daily activities of 53.4% of the patients, the work life of 50.7% and the social life of 46.6% of the patients were moderately affected by hemorrhoids. Before and after interventional hemorrhoid treatment, patients mostly encountered problems such as bleeding, constipation, itching, pain in the anal region, painful/difficult defecation. . The highest and lowest total scores of the patients on the 25th day of RRYQ before and after
treatment were 192-78, respectively; It was determined as 202-92. In the study, it was determined that the quality of life of the patients on the 25th day after the treatment increased compared to the pre-treatment.
Conclusion: In the study, it was determined that the patients who underwent interventional hemorrhoids experienced many problems and these problems affected the quality of life of the patients. In line with the results of the research, it is recommended to provide counseling services by monitoring the problems that patients may experience after interventional hemorrhoid treatment and to solve these problems after discharge, to develop educational materials and to use them in patient education, and to conduct multicenter studies with larger samples to determine their quality of life.

Girişimsel Hemoroid Tedavisi Uygulanan Hastaların Yaşadıkları Sorunların ve Yaşam Kalitelerinin Belirlenmesi

Abstract

Amaç: Bu araştırma, girişimsel hemoroid tedavisi uygulanan hastaların yaşadıkları sorunların ve yaşam kalitelerini belirlemek amacıyla tanımlayıcı olarak yapılmış olup, çalışma sonucunda girişimsel hemoroid tedavisi sonrası meydana gelecek sorunların önlenmesi ve hastaların yaşam kalitelerinin daha az etkilenmesine yönelik hemşirelik bakımının uygulanmasında yol gösterici olacağı ve literatüre katkı sağlayacağı düşünülmektedir.
Gereç ve Yöntem: Araştırmanın örneklemine Ankara'da bir üniversitenin Sağlık Araştırma ve Uygulama Merkezi Genel Cerrahi ve Proktoloji biriminde girişimsel hemoroid tedavisi uygulanan ve araştırmaya katılmayı kabul eden 73 hasta alınmıştır. Verilerin toplanmasında sosyodemografik soru formu, tedavi sonrası komplikasyonları belirlemeye yönelik soru formu ve Roll Royce Yaşam Kalitesi Ölçeği kullanılmıştır. Çalışmadan elde edilen verilerin analizinde; sayı, yüzde, normal dağılıma sahip iki bağımlı grubun karşılaştırmalarında “Paired Sample-t” test (t-tablo değeri) ve normal dağılıma sahip olmayan iki bağımlı grubun karşılaştırmalarında “Wilcoxon” test (Z-tablo değeri) kullanılarak değerlendirilmiştir.
Bulgular: Hastaların %53,4’ünün günlük aktivitelerinin, %50,7’sinin iş hayatının ve %46,6’sının ise sosyal hayatının hemoroid nedeniyle orta derecede etkilendiği saptanmıştır. Girişimsel hemoroid tedavisi öncesi ve sonrası hastalar en çok kanama, konstipasyon, kaşıntı, anal bölgede ağrı, ağrılı/zor defekasyon gibi sorunlarla karşılaşmış olup, genellikle bu sorunların tedavi sonrası 1. günde artış gösterdiği diğer günlerde ise azaldığı ancak 25. günde hala devam ettiği saptanmıştır. Hastaların RRYKÖ’nin tedavi öncesi ve tedavi sonrası 25. günde toplam alınan en yüksek ve en düşük puanlar sırasıyla 192-78; 202-92 olarak saptanmıştır. Çalışmada hastaların tedavi sonrası 25. gündeki yaşam kalitelerinde tedavi öncesine göre artış olduğu belirlenmiştir.
Sonuç: Çalışmada girişimsel hemoroid uygulanan hastaların birçok sorun yaşadıkları ve bu sorunların ise hastaların yaşam kalitelerini etkilediği saptanmıştır. Araştırma sonuçları doğrultusunda girişimsel hemoroid tedavisi sonrası hastaların yaşayabilecekleri sorunlar ve taburculuk sonrası bu sorunları çözümleyebilmelerine ilişkin telefonla izlem yapılarak danışmanlık hizmeti verilmesi, eğitim materyallerinin geliştirilerek hasta eğitiminde kullanılması ve yaşam kalitelerinin belirlenmesine yönelik daha geniş örneklemli ve çok merkezli çalışmaların yapılması önerilmektedir.
