The investigation of anesthesia’s awareness on eras protocol applications in bariatric surgery

Abstract

Aim:The purpose of our study is to research the awareness about the ERAS protocol which includes factors about optimum perioperative care in bariatric surgery, based on the evidence of advanced perioperative protocols. Material and Methods:After obtaining the required ethics committee permit, a study was planned in the form of a questionnaire for anesthesiologists and specialists. The questionnaire contains 27 questions in total. Prepared forms were delivered to anesthesiologists and their assistants by Turkish Society of Anesthesiology and Reanimation electronic mail group or direct meeting.The analysis of the data was done by using descriptive statistical methods and the comparison of qualitative data with the SPSS 23.0 program. p <0.05 was considered significant. Results: In the study, there was a statistically significant difference between the anesthesia experiences in the questions of cardiac risk assessment, cardiac index, and approach in patients using alcohol, monitoring the depth of anesthesia (BIS) and muscle relaxant, use of nasogastric tube, and which fluid was used frequently (p <0.05) has been found. It was found that those who followed the preoperative fasting protocol also had applications for post-op nausea and vomiting. It was found that early mobilization rates were higher in patients who applied thromboprophylaxis than those who did not (p <0.05). Conclusion: It was found out that the ERAS protocol is not applied enough in bariatric surgery and it was also found out that these applications differ with the physician’s experience. In general, we found that anesthesiologists followed general ERAS guideline. They should increase their awareness about different surgical approaches like bariatric surgery.

Anestezistlerin bariatrik cerrahide eras protokolü uygulamalarıyla ilgili farkındalığının araştırılması

Abstract

Amaç: Çalışmamızın amacı anestezistler arasında bariatrik cerrahide optimum perioperatif bakım ile ilgili kanıta dayalı gelişmiş perioperatif protokol unsurları içeren ERAS protokolünün farkındalığını araştırmaktadır. Gereç ve Yöntemler: Etik kurul onayı alındıktan sonra anesteziyoloji kliniği asistan ve uzmanlarına yönelik anket çalışması olarak planlanmıştır. Çalışmamıza hastanemizdeki anestezi kliniği asistan ve uzmanlarının yanında, Türk Anesteziyoloji ve Reanimasyon Derneği’nin katkılarıyla birçok farklı üniversite ve eğitim araştırma hastanesindeki anestezi uzman ve asistanları da katılmıştır. Anketimiz 27 sorudan oluşmaktadır ve toplam 245 anestezi doktoru doldurmuştur. SPSS 23.0 paket programı ile gerekli testler yapılarak değerlendirilmiştir. Bulgular: Yaptığımız çalışmada ERAS uygulamalarının anestezi deneyimine göre karşılaştırılmasında; kardiyak risk değerlendirilmesi, kardiyak indeks ve alkol kullanan hastalardaki yaklaşım, BiS ve TOF kullanımının, nazogastrik tüp kullanımı ve sıvı olarak en sık hangisinin kullanıldığı sorularında anestezi deneyimleri arasında istatistiksel olarak anlamlı bir fark olduğu (p<0,05) bulunmuştur. Preoperatif açlık protokolü uygulayanların post-op bulantı-kusma için de uygulamaları olduğu saptanmıştır. Tromboprofilaksi uygulayanlarda erken mobilizasyon uygulanma oranlarının uygulamayanlara göre daha yüksek olduğu bulunmuştur (p<0,05). Sonuç: Anestezistlere uyguladığımız bu ankette ERAS protokolünün bariatrik cerrahide yeteri kadar uygulanmadığı, anestezistlerin deneyimiyle de bu uygulamaların farklılık gösterdiği ortaya çıkmıştır. Her geçen gün önemi kanıtlanan ERAS protokolünün bariatrik cerrahide uygulanması için daha çok bilgilendirmeye ihtiyaç olduğu sonucuna varıldı.
