KORYOKARSİNOM: OLGU SUNUMU

Abstract

ABSTRACT: The aim is to present a rare and aggressive choriocarcinoma case. Choriocarcinoma; It is one of the most aggressive gestational trophoblastic neoplasms consisting of anaplastic trophoblastic tissues with vascular invasion, characterized by pronounced bleeding and necrosis. The 47-year-old patient with nausea, vomiting, and abdominal pain was referred to our clinic with a high β-hCG. A 16-week amenorrhea, hyperemesis and hyperthyroidism clinic was diagnosed with probe curettage and choriocarcinoma, and metastasis was detected in thoracic computed tomography, and brain metastasis was not detected in brain magnetic resonance imaging. In all abdominal magnetic resonance imaging, there was no pathology in the upper abdomen, but a uterus-located mass was present. Since the patient was older and not prompted for children, hysterectomy was performed before chemotherapy and directed to medical oncology in order to reduce the risk of recurrence. The patient is followed up in remission after the EMA / CO (11Cour) protocol. ÖZET : Amaç nadir görülen ve agresif seyreden bir koryokarsinom olgusunun sunulmasıdır. Koryokarsinom; belirgin kanama ve nekrozla karakterize, vasküler invazyon içeren anaplastik trofoblastik dokulardan oluşan, en agresif gestasyonel trofoblastik neoplazilerden biridir. 47 yaşında bulantı, kusma ve karın ağrısı şikayetleri bulunan hasta β-hCG yüksekliği ile kliniğimize refere edilmiştir. 16 haftalık amenore, hiperemezis ve hipertiroidi kliniği bulunan hastanın yapılan tetkik ve araştırmalar sonucunda probe küretaj ile koryokarsinom tanısı konulmuş ve toraks bilgisayarlı tomografide akciğerde metastaz saptanmıştır, beyin manyetik rezonans görüntülemede metastaz saptanmamıştır. Tüm abdomen manyetik rezonans görüntülemede üst abdomende patoloji saptanmamakla birlikte uterus yerleşimli kitle mevcuttu. Hastanın ileri yaş olması, çocuk istemi olmaması nedeniyle nüks riskini azaltmak amacıyla kemoterapi öncesi histerektomi uygulandı ve medikal onkolojiye yönlendirildi. Hasta EMA/CO (11Kür) protokolü sonrası remisyonda olarak takip edilmektedir.
