HEMATOPOİETİK KÖK HÜCRE TRANSPLANTASYONU ADAYLARINDA KARDİYOPULMONER UYGUNLUK DÜZEYİ VE İLİŞKİLİ FAKTÖRLER

Abstract

Amaç:
Çalışmamızın
amacı, hematopoietik kök hücre transplantasyonu (HKHT) adaylarında kardiyopulmoner
uygunluk (KPU) düzeyinin belirlenmesi ve KPU düzeyi ile ilişkili faktörlerin
araştırılmasıdır.

Yöntem: 18-65 yaşları arasında, ilk kez HKHT için Hacettepe
Üniversitesi Kemik İliği Transplantasyonu Servisi’ne yatırılan 76 birey (erkek:
45(% 59,2), kadın: 31(% 40,8) dahil edildi. 6 Dakika Yürüme Testi (6 DYT), KPU
düzeyini belirlemede kullanıldı. Kavrama ve periferal kasların kuvveti elde
tutulan dinamometre ile ölçüldü. Alt ekstremite kas enduransı 30 saniye otur
kalk testi ile değerlendirildi. Fonksiyonel mobilite Zamanlı Kalk Yürü Testi-3
metre ile ölçüldü. Yorgunluk Şiddet Ölçeği, Hastane Anksiyete ve Depresyon
Ölçeği ve
Avrupa Kanser Araştırmaları ve Tedavi
Organizasyonu–Yaşam Kalitesi Anketi sırasıyla yorgunluk, emosyonel durum ve
yaşam kalitesini değerlendirmede kullanıldı.

Sonuçlar:
Bireyler norm veriler ile karşılaştırıldığında istatistiksel olarak daha düşük
6 DYT mesafesine sahipti (ortalama fark: 69,27 m, p˂0,001) ve kadınlar
erkeklere göre daha düşük KPU düzeyindeydi (p=0,03). Ayrıca, geçmişte egzersiz
alışkanlığı bulunan bireyler egzersiz yapmayanlara göre daha yüksek KPU düzeyindeydi
(ortalama fark: 61,89 m, p=0,03). Yaş, periferal kas kuvveti ve fonksiyonel
mobilite düzeyi orta-yüksek derecede 6 DYT mesafesi ile koreleydi (p˂0,01).







Tartışma:
HKHT için hastaneye yatırılan bireylerde KPU’ta bozukluk vardır ve bu kişisel
ve klinik faktörler ile ilişkilidir. HKHT öncesi ileri yaşta olan, egzersiz
geçmişi olmayan, düşük periferal kas kuvveri ve fonksiyonel mobiliteye sahip
hastalar dikkatli takip edilmeli ve KPU düzeyini korumak veya artırmak için
egzersiz tedavisine yönlendirilmelidir.
