Vajinal Kaf Prolapsus Tedavisinde Cerrahi Kit Kullanımı

Abstract

Bu yazıda amacımız, vaginal kaf prolapsusu nedeniyle bir olguya uyguladığımız posterior intravajinal slingoplasti operasyonunu sunmak, cerrahi tekniği ve literatürü gözden geçirmektir. Altmış beş yaşında olan hasta yaklaşık 6 aydan beri ayakta dururken ve ıkındığında vajenden eline kitle gelmesi, idrarını tam boşaltamama ve idrar yapmada zorluk şikâyetiyle ürojinekoloji polikliniğimize başvurdu. Olguya evre III vajinal kaf prolapsusu tanısı kondu. POP-Qsınıflama sistemine göre Aa = -2, Ba = -2, C = +7, Ap= +3, Bp= +4, TVL= 7.5, gh = 4.5, pb= 5.5 idi. Olguya vajinal kaf prolapsusu endikasyonuyla makroporlu, monofilament polipropilen meş cerrahi kiti ile posterior intravajinal slingoplasti uygulandı. Vajinal kaf prolapsusu tedavisinde yapılabilecek en uygun cerrahi, hastaya bütün seçenekler kür ve komplikasyon oranları ile birlikte sunularak, hasta ile ortak bir kararla seçilmelidir. Vajinal kaf prolapsusunda cerrahi kit kullanımının etkinliği ve güvenilirliği doğrulayabilmek için uzun takip süreli ve geniş ölçekli randomize kontrollü çalışmalara ihtiyaç bulunmaktadır. Surgical Kit Usage for the Treatment of Vaginal Vault Prolapse: Case Report

Abstract

We aimed to present a case of posterior intravaginal slingplasty for vaginal vault prolapse and review the surgical technique and literature. 65- year-old female patient presented to our urogynecology outpatient care clinic with vaginal mass protrusion during straining and standing for the last 6 months. She was diagnosed as stage III vaginal vault prolapse. According to POP-Q.staging system classified as Aa = -2, Ba= -2, C = +7, Ap= +3, Bp= +4, TVL= 7.5, gh = 4.5, and pb= 5.5. Posterior intravaginal slingoplasty was executed on the case with the indication of vaginal vault prolapse using macropore, monofilament polypropylene mesh surgical kit. Surgical management in vaginal vault prolapse should be chosen in consensus with the patient by assessing the cure and the complication rates of all alternatives. Randomized large scale controlled studies with long followup is needed to confirm the efficiency and reliability of surgical kit use in vaginal vault prolapse
