PRİMER GÖĞÜS DUVARI TÜMÖRLERİ

Abstract

Vücudun tüm primer tümörlerinin % 2’sini, toraksa ait tümörlerin ise yalnızca % 5’ini oluşturan primer göğüs duvarı tümörleri yumuşak dokular, kıkırdak veya kemik yapılardan köken alan nadir kitlelerdir. Bu kitlelerde malignite oranı yaklaşık % 60-80 civarındadır. Primer göğüs duvarı tümörü tanısı almış 36 olgunun radyolojik bulgularını gözden geçirmek, primer göğüs duvarı kitlelerinin demografik dağılımını ve bu kitlelere yaklaşımı ortaya koymak amaçlanmıştır. Ocak 2007 ve Mart 2010 yılları arasında primer göğüs duvarı tümörü tanısı alan 36 olgunun görüntüleme bulguları retrospektif olarak değerlendirildi. Otuz altı olgunun 19’u kadın, 17’si erkek, yaşları 16 ile 77 arasında, ortalama 47.5 ± 16.5 idi. Tüm olgulara bilgisayarlı tomografi ve mag-netik rezonans değerlendirmeyi takiben lezyonlardan histopatolojik tanı amacıyla biyopsi yapıldı. Olguların 6’sı kondrosarkom, 4’ü dermatofibrosarkom protüberans, 3’ü plazmasitom, 3’ü osteo-kondrom, 2’si fibröz displazi, 2’si Ewing sarkomu, 2’si primitif nöroektodermal tümör, 3’ü desmoid tümör, 2’si elastofibrom, 2’si Hodgkin lenfoma, diğerleri birer olgu olarak rabdomiyosarkom, malign fibröz histiyositom, osteosarkom, enkondrom, nörofibrom, andiferansiye malign tümör ve dev hücreli tümör tanısı aldı. Çalışmamızda primer göğüs duvarı tümörü saptanan 36 olgunun klinik ve radyolojik bulguları, demografik dağılımını ve bu kitlelere yaklaşım literatür bilgileri ışığında değerlendirilmiştir. Klinik bulgularla birlikte MRG ve BT bulgularının değerlendirilmesi, primer göğüs duvarı kitlelerinde ma-lignitenin ekarte edilebilmesinde ve kitlelerin ayırıcı tanısında yol gösterici olacaktır. PRIMARY CHEST WALL TUMORS

Abstract

Primary chest wall tumor is rare that represents about 2% of all primary malignancies and 5% of all thoracic neoplasms. It encompasses tumors of various origins, including bone and cartilage, soft tissue such as muscle, vessel, and nerve. 60%-80 of primary chest wall masses is malignant. The aim of this study is to analyze the demographic presentation and the management of 36 primary chest wall tumors From January 2007 to March 2010 imaging findings of 36 patients with the diagnosis of primary chest wall tumors were retrospectively evaluated. Nineteen of 36 patients were female, 17 of them were male, age ranges from 16 to 77 (mean 47.5±16.5). All patients underwent biopsy and histopathologic diagnoses were made following computed tomography and magnetic resonance imaging. The diagnoses of six of the 36 cases were chondrosarcoma, 4 were dermatofibrosarcoma protuberance, 3 were plasmacytoma, 2 were fibrous dysplasia, 2 were Ewing’s sarcoma, 2 were primitive neuroectodermal tumor, 3 were desmoid tumor, 2 were elastofibroma, 2 were Hodgkin’s lymphoma, other tumors as one case was rhabdomyosarcoma, malignant fibrous histiocytoma, osteosarcoma, enchondroma, neurofibroma, undifferentiated malignant tumor and giant cell tumor. Herein, the clinical, pathologic, and radiological findings of primary chest wall tumors were investigated in the light of literature. Both clinical findings and CT, MR findings may help to exclude a diagnosis of malignancy in primary chest wall tumors and also in differential diagnosis of the masses.
