CERRAHİ UZMANLIK EĞİTİMİNDE; AMELİYAT RAPORU YAZIMI VE STANDARDİZASYON-1

Abstract

Ameliyat notları; klinik, akademik ve mediko-legal kayıtların önemli parçalandır. Dünyanın her yerindeki tıbbi ve etik kurullar; doktorların doğru, kapsamlı ve güncel tıbbi kayıtlar tutmasını tavsiye ederler. Uygun bir ameliyat raporunun oluşturulması sadece iyi bir uygulama değil, aynı zamanda bir zorunluluktur. Birçok çalışma, ameliyat notlarının sıklıkla çelişkili ifadeler içerdiğini ve temel ayrıntılarda bile eksiklik olduğunu gösterilmiştir. Dolayısıyla cerrahi raporlar mediko-legal malpraktis durumlarında, sıklıkla doktorların aleyhine delil olarak kullanılmaktadır. Çeşitli eğitim kurumlan ve uzmanlık dernekleri, iyi bir tıbbi uygulama için kendi görüşlerini bildirmiştir. Ancak halen bu konuda bir fikir birliği yoktur. İçeriğin standardize edilmesi; verilerdeki belirsizlik ve ihmali azaltarak, güvenliği artırır. Cerrahi eğitim, denetim ve özel kontrol listesi kullanımı daha kaliteli ameliyat notları üretecektir. Önceki çalışmalar, hatırlatıcı notların veya proforma temelli kılavuzların daha iyi ameliyat raporu oluşturulmasına yardımcı olduğunu göstermiştir. Ameliyat notlarının en etkili kayıt yöntemi bir operatif veritabanı oluşturmaktır. Mükemmel bir veritabanı oluşturmanın; okunaklı bir ameliyat raporu elde etme, otomatik kodlama ve otomatik evreleme avantajları vardır.

SURGERY REPORT WRITING AND STANDARDIZATION IN SURGICAL SPECIAL EDUCATION- 1

Abstract

Operation notes are an important parts of medical records for clinical, academic and medicolegal reasons. Medical and ethic councils around the world advise doctors to keep accurate, comprehensive, contemporaneous medical notes. Providing an appropriate operation note is not only a good practice, it is a requirement. Many studies have shown that operative notes often have contradictive statements and lack basic details. As a result surgical notes are often produced as evidence against doctors in medico-legal malpractice cases. Several colleges and specialist societies and the medical defence organisation have published their own perspectives for good medical practice. However there was not a consensus at this issue for today. Standardisation of content will improve safety by reducing opportunities for ambiguity or omission of data. Surgical education, audit and the use of a specific checklist produces better quality operation notes. Previous studies have shown that aide-memoire or proforma based guidelines help produce better operations notes. The most effective method of recording operation notes would be an operative database. The advantages of excellent database include a printed legible operative record, automatic coding and automatic grading.
