Mastektomi Sonrası Yara İyileşmesinde Kan Transfüzyonunun Rolü Var mı?

Abstract

Çalışmamız perioperatif kan transfüzyonu yapılmasının; postoperatif yara iyileşmesinde rolü olup olmadığını incelemek amacı ile düzenlendi. Meme kanseri dolayısıyla modifiye radikal mastektomi ve Level 1-2-3 aksiller lenf nodu disseksiyonu yapılmış 126 hastada kan transfüzyonu ve yara iyileşmesi arasındaki muhtemel ilişki flep iskemi-nekrozu, yara yeri enfeksiyonu, total drenaj
miktarı ve total drenaj süresi parametreleriyle incelendi. Yara yeri enfeksiyonu insizyon hattında eritem-ödem-akıntı-pürülan dren getirisi göz önüne alınarak değerlendirildi. Flep iskemi ve nekrozu belirleme ve ölçmede saydam kağıt metodu kullanılmıştı. Hastalar kan transfüzyonu yapılmış (Grup 1: 72 hasta) ve kan transfüzyonu yapılmamış (Grup 2: 54 hasta) olarak 2 gruba ayrıldı. Bu iki
grup hasta dosyaları günlük takip kayıtları kullanılarak retrospektif olarak incelendi. Transfüzyon yapılan ve yapılmayan gruplar arasında yukarıdaki parametreler açısından anlamlı fark bulunmadı. Metabolik olarak kesin endikasyonu bulunan şartlar dışında kan transfüzyonu açısından rölatif endikasyonlar ortaya çıktığında, yara iyileşmesine katkıda bulunmayabileceği de göz önüne alınarak kan transfüzyonu yapılması açısından oldukça ihtiyatlı davranmanın hastalığın prognozuna olumlu katkı sağlayabileceği görüşündeyiz.
