Siyatik Sinir Paralizisi, Süperfisial Femoral Arter ve Derin Femoral Ven Yaralanmasının Bir Arada Görüldüğü Açık Femur Diafiz Kırığı Olgusu

Abstract

Femur diafiz kırıklarına eşlik eden vasküler patolojilere literatürde nispeten sık rastlanmaktadır. Buna karşın vasküler patolojiye ek olarak periferik sinir patolojilerinin beraber görüldüğü vakalara nadiren rastlanılmaktadır. Sunacağımız olgumuz; 20 yaşında bayan hasta, araç dışı trafik kazası sonrası acil ünitemize getirildi. Yapılan fizik muayene ve radyodiagnostik inceleme sonrası
başlangıçta, Gustilo & Anderson sınıflamasına göre Tip-IIIa açık femur kırığı tanısı konuldu. Tekrarlanan muayenelerinde yüzeyel femoral arter, derin femoral ven yaralanması ve siyatik sinir paralizisi tespit edilerek Gustilo & Anderson sınıflamasına göre Tip-IIIc tanısı ile operasyona alındı. Geçici stabilizasyonu takiben açık olarak vasküler onarım yapıldı. Post op 3. haftada EMG tetkiki ve femur kırığının definitif tedavisi yapıldı.
