Akut Akut Faz Reaktanların Disk Hernisi İle İlişkisi

Abstract

Disk hernileri ve ultrasensitif CRP düzeyi arasında ilişki olduğu ileri sürülmektedir. Bu çalışmada, bel ağrısı şikayeti ile başvurmuş olan hastalarda fizik muayene, bilgisayarlı tomografi ve akut faz
reaktanları arasındaki ilişkinin saptanması amaçlanmıştır. Çalışmaya bel ağrısı şikayeti ile başvuran 103 hasta dahil edildi. Fizik muayene sonrası kanda beyaz küre, sedimantasyon, romatoid
faktör, CRP tetkikleri yapıldı. Lomber vertebra tomografileri çekildi. Tomografi bulguları normal (grup 1) ve anormal (grup 2) olan hastalar karşılaştırıldı.
Grup 1 16 hastadan oluşmakta olup, beyaz küre 6560±1970/mm3, CRP 5.1±6.3 mg/L, sedimantasyon 9.7±6.5 mm/st, RF 9.8±0.4 IU/ml bulundu. Grup iki 87 hastadan oluşmakta olup,
beyaz küre 7180±1660/mm3, CRP 4.7±3.6 mg/L, sedimantasyon 12.5±8.3 mm/st, RF 10.5±7.3 IU/ml bulundu. Grupların laboratuar parametreleri arasında fark bulunmadı. Duyu kaybı bulunan hastaların beyaz küre sayısı (p=0.017), refleks bozukluğu bulunan hastaların ise sedimantasyon değeri (p=0.002), normal grup hastalardan yüksek tespit edildi.
Disk hernisi ile nefelometrik yöntemle bakılan CRP arasında ilişki saptanmamıştır. Duyu kaybı bulunan hastalarda beyaz küre sayısı, refleks bozukluğu tespit edilen hastalarda ise sedimantasyon
yüksek bulunmuştur. Bu durum disk hernilerinde akut faz reaktanlarının radyolojik bulgulardan ziyade muayene bulguları ile paralellik gösterdiğini düşündürmektedir.

Abstract
High level of ultra sensitive CRP is known to be present in disc hernia. The relationship between physical findings, blood acute phase reactant levels and computed tomographic findings in disc hernia are studied. In the study 103 patients were included. After physical examination, leukocyte count, sedimentation rate, romatoid factor and CRP
levels are measured. Two groups (group 1 normal computed tomographic findings and group 2 (abnormal computed tomographic findings) are compared. Group 1 was composed of 16 patients with leukocyte count of 6560+/-1970/mm3, CRP of 5.1+/-6.3 mg/L, sedimentation rate of 9.7+/-6.5 mm/hr and romatoid factor level of 9.8+/-0.4 IU/ml. Group 2 was composed of 87 patients with leukocyte count of 7180+/- 1660/mm3, CRP of 4.7+/-3.6 mg/L, sedimentation rate of 12.5+/-8.3 mm/hr and romatoid factor level of 10.5+/-7.3 IU/ml. There was no
difference in laboratory findings at two groups. Leukocyte count in patients with sense disturbance (p=0.017) and sedimentation rate in patients with diminished reflex (p=0.002) were higher than normal group.
There was no relation found between disc hernia and CRP levels observed by nephelometric method. Higher leukocyte count in patients with sense disturbance and higher sedimentation rate in
patients with diminished reflex were established. It was thought that the blood acute phase reactant levels are in correlation with the physical findings in disc hernia rather than radiological findings.
