Effect of Presence of Pseudoexfoliation on Postoperative Corneal Edema and IOP After Cataract Surgery with Phacoemulsification

Abstract

The Purpose of the study is to evaluate the effect of presence of pseudoexfoliation (PEX) on postoperative corneal edema (PCE) and IOP after cataract surgery with phacoemulsification. In this study, cases after cataract surgery with phacoemulsification of which 56 cases with PEX (study group) and 63 cases without PEX (control group), were studied. Effective phaco time and phaco time were used as phaco surgery parameters. Preoperative and postoperative lOPs were recorded in all cases. Grade of PCE were also recorded in all cases. There was no significant difference between study group and control group in preoperative and postoperative IOP (p=0.66, p=0.99 respectively). Although cases in the study group have a 2.2+0.4 min mean phaco time and 0.5+0.1 min mean effective phaco time, mean phaco time was 2.0+0.9 min and mean effective phaco time was 0.4+0.1 min in control group. Differences between these two groups regarding to phaco time and effective phaco time were also insignificant (p=0.304 and p=0.725 respectively). PCE was recorded as grade 1 in 32 cases (57.1%) and grade 2 in 24 cases (42.8%) in study group. On the other hand these figures were 34 cases (53.9%) and 29 cases (46.0%) in control group. Mean phaco time was 1.9+0.8 min in cases with grade 1 PCE and was 2.4+0.9 min in cases with grade 2 PCE. There is a statistically significant difference between cased with grade 1 and grade 2 PCE (p=0.001). Similarly mean effective phaco time was 1.9+0.8 min in cases with grade 1 PCE and was 2.4+0.9 min in cases with grade 2 PCE. There is a statistically significant difference between cased with grade 1 and grade 2 PCE (p=0.001). A significant change between mean preoperative and postoperative IOP was not observed in cased with PEX. PEX did not appear as a major risk factor for PCE. However, longer durations of phaco time and effective phaco time poses a significant PCE risk.

Psödoeksfoliasyon Varlığının Fakoemülsifikasyon ile Katarakt Cerrahisi Sonrası Kornea Ödemine ve Göz İçi Basıncına Etkisi

Abstract

Çalışmamızda Fakoemülsifikasyon ile katarakt cerrahisi sonrası gelişebilen kornea ödemi ve göz içi basınç (GİB) değişikliklerine psödoeksfoliasyon (PE) varlığının etkisinin incelenmesi amaçlanmıştır. Fakoemülsifikasyon ile katarakt cerrahisi geçiren ve PE’si olan 56 olgu (çalışma grubu) ile PE’si olmayan 63 olgu (kontrol grubu) çalışmaya dahil edildi. Fako parametreleri olarak ortalama (ort.) fako zamanı ve efektif fako zamanı kaydedildi. Tüm olguların ameliyat öncesi ve sonrası GİBTarı ölçüldü. Ameliyat sonrası kornea ödemi dereceleri kaydedildi. Ameliyat öncesi ve sonrası GİB değerleri bakımından her iki grup arasında anlamlı fark yoktu (p=0.66, p=0.99). Çalışma grubunda fako zamanı ort. 2.2±0.4 dk. ve efektif fako zamanı ise ort. 0.5±0.1 iken kontrol grubunda fako zamanı ort. 2.0±0.9 dk. ve efektif fako zamanı ise ort. 0.4±0.1 olarak tespit edildi. Her iki grup arasında fako zamanı ve efektif fako zamanı bakımından anlamlı bir fark yoktu (p=0.304, p=0.725). Çalışma grubunda olguların 32’sinde (%57.1) 1.derece, 24’ünde (%42.8) 2. derece ödem saptanırken kontrol grubunda bu değerler sırasıyla 34 (%53.9) ve 29 (%46) olgu idi. 1. derece kornea ödemi gelişen olguların fako zamanı ort. 1.9±0.8 dk. iken 2. derece ödem gelişen olgularmki ort. 2.4±0.9 dk. ve fark istatistiksel olarak anlamlıydı (p=0.001). 1. derece kornea ödemi gelişen olguların efektif fako zamanı ort. 0.2±0.1 dk. iken, 2. derece ödem gelişen olgularınki ort. 0.3±0.1 dk. ve fark istatistiksel olarak anlamlıydı (p=0.001). PETi olgularda ameliyat sonrası GİB’ında önemli bir değişiklik izlenmemiştir ve kornea ödemi gelişiminde PE bir risk faktörü olmadığı düşünülmektedir. Fako cerrahisi sonrası kornea ödemi gelişiminde fako zamanı ve efektif fako zamanı önemli bir risk faktörü olarak tespit edilmişlerdir.
