The Relation Between Iron Deficiency Anemia and HbAlc Levels

Abstract

In this study; HbAlc levels of iron deficiency anemic patients were measured and compared with after treatment values. Sixty five patients with iron defiency anemia and fourty patients as a control grup who were not iron deficient were enrolled. Patients who had taken iron deficiency treatment last six months, who were diabetic or had impaired fasting glucose or having a concomitant disease were excluded from the study. Complete blood count, serum iron, iron binding levels, transferin saturation, ferritin, HbAlc levels, fasting glucose levels were recorded. The anemic patients who were given iron treatment were reevaluated after 6 months of therapy. Mean HbAlc levels of the case group was statistically significantly higher compared to the control group before iron treatment (p<0,001). After iron treatment, mean HbAlc levels was found to be similar between the case and control groups (p=0,641). Our data provide evidence that HbAlc levels might be misleading during the efforts to pursue glycemic control at patients who are both diabetic and iron deficient. For this reason it would be profitable to treat iron deficiency first before planning any tests or long term treatment modalities which are based on HbAlc levels.

Demir Eksikliği Anemisi ve HbAlc Düzeyi Arasındaki ilişki

Abstract

Bu çalışmada, demir eksikliği anemili olgularda HbAlc düzeyleri ölçülerek tedavi sonrası değerlerle karşılaştırıldı. Demir eksikliği anemisi olan 65 olgu ve kontrol grubu olarak demir eksikliği anemisi olmayan 40 olgu çalışmaya dahil edildi. Son 6 ay içinde demir tedavisi alan, diyabetes mellitus veya bozulmuş açlık glukozu olan, eşlik eden başka sistemik hastalığı olanlar çalışma dışı bırakıldı. Olguların tamamının hemogram, serum demir, demir bağlama düzeyleri, transferrin saturasyonları, ferritin, HbAlc, açlık kan glukozu düzeyleri kaydedildi. Anemili olguların tedavi başlandıktan 6 ay sonra kontrolleri yapıldı. Demir eksikliği anemisi grubunda tedavi sonrasında HbAlc ortalamalarında tedavi öncesine göre anlamlı azalma gözlendi (p<0,001). Tedavi sonrasında demir eksikliği anemisi grubu ile kontrol grubunun HbAlc düzeyleri arasında anlamlı fark yoktu. (p=0,641). Özellikle diyabetik ve demir eksikliği anemisi olan olgulann HbAlc düzeyleri glisemik kontrolün takibi açısından yanıltıcı sonuçlara neden olabilir. Bu nedenle HbAlc temel alınarak yapılacak herhangi bir tetkik ya da tedavi uygulamasından önce demir eksikliğinin düzeltilmesi uygun olacaktır.
