The Treatment Procedures Of Humerus Shaft Fractures

Abstract

Between September 1998 and November 2002, 62 cases with humerus shaft fracture, 48 male and 14 female, mean ages 32.8 years (15-67) that have been treated in our clinic were evaluated . Mean follow up was 18.5 months (3-38). Twenty-eight of the cases were treated surgically, and 34 of them by conservative methods. Twelve of the patients who were initially treated by conservative methods underwent surgery due to delayed union. Isolated humerus fractures were found in 33 cases and concomitant injuries in 29 cases. Four cases, who had primary radial nerve repair, the pronator teres was transferred to the extensor carpi radialis brevis the for internal splint. The union of the fracture was accomplished in 12 weeks in the operated group and in 10 weeks in the conservative group. In twelve cases, that were treated by conservative means, the union could be accomplished by surgical intervention. With pronator teres internal splint, cases who had primary radial nerve repair, could use their hands earlier with less prolonged use of external splint until the nerve regeneration accomplished. Humerus Cisim Kırıklarında Tedavi Yöntemleri

Abstract

Bu çalışmamızda, humerus diyafiz kırığı nedeniyle, kliniğimize başvuran olgulara uyguladığımız konservatif ve cerrahi tedavi sonuçlarımızı irdelemeyi amaçladık. Eylül 1998 ile Kasım 2002 tarihleri arasında kliniğimize humerus diyafiz kırığı ile başvuran 62 hasta, retrospektif olarak değerlendirildi. Olguların 48'i erkek ve 14'ü bayan olup, yaş ortalamaları 32.8 (15-67yıl) idi. Ortalama 18.5 (3- 38ay) izlenen olguların 28'i primer cerrahi yöntemlerle, 34'ü konservatif yöntemlerle tedaviye başlanmış; ancak çeşitli nedenlerle bunların 12'sinde, daha sonra cerrahi müdahale gerekmiştir. 33 olguda izole kırık varken, 29'unda ek patolojiler bulundu.Radial sinir primer tamiri yapılan dört olguya pronator teres'in ekstensör karpi radialis brevis'e transferi yapılarak internal splint ameliyatı yapıldı. Cerrahi tedavi edilen 28 olguda kaynama ortalama 12 haftada elde edildi. Konservatif tedavide 34 olgudan 22'sinde 10 haftada yeterli kaynama kaydedilirken, 12 hastada cerrahi müdahale sonrası kaynama elde edildi. Pronator teres internal splint ameliyatı yapılan hastalarda uzun dönemde sinir rejenerasyonu gelişene kadar, splint kullanım süresi kısalmış ve el daha erken fonksiyonel hale getirildiğini düşünüyoruz.
