Sinonasal Malignant Fibrous Histiocytoma

Abstract

Although malignant fibrous histiocytoma is one of the most commonly encountered malignant soft tissue tumor, head and neck presentation is extremely rare. The aim of this article is to present the results of magnetic resonance imaging (MRI) of a case with sinonasal malignant fibrous histiocytoma. A 59 years old male patient was referred to a tertiary medical center with a mass in the midface, nasal obstruction, epistaxis and anosmia. In MRI assesment, the involvement of the nose, bilateral nasal cavities, anterior ethmoids, both frontal sinuses and right maxillary sinus was observed. The mass having decreased signal intensity in T1W images and increased signal intensity in T2W was also showing an apparent heterogenous contrast enhancement. In MRI sections, the invasion into the superior orbit and anterior cranial fossae was determined. Though it is rare, the malignant fibrous histiocytoma should be remembered in the differential diagnosis of sinonasal masses. MRI assesment is valuable in the detection of the involvement of the surrounding tissues. Key words: Malignant fibrous histiocytoma, Sinonasal, MRI

Sinonazal Malign Fibröz Histiositom’un MR Bulguları+

Abstract

Malign fibröz histiositom erişkinlerde en sık görülen malign yumuşak doku tümörü olmasına rağmen baş- boyun bölgesi nadir yerleşim yerlerinden biridir. Bu çalışmada sinonazal yerleşimli massif malign fibröz histiositom olgusunda manyetik rezonans (MR) görüntüleme (bulgularının sunulması amaçlanmaktadır. Yüz orta hattında kitle, giderek artan burun tıkanıklığı, burun kanaması ve anosmi semptomları olan 59 yaşında erkek hasta konvansiyonel MR ile incelendi. MR'da burun, bilateral nazal kavite, anterior etmoid ve frontal sinüsleri dolduran ve sağ maksiller sinuse de uzanan orta hat yerleşimli, yaklaşık 65x80x95mm boyutlu solid tümöral kitle lezyonu izlendi. T1 A görüntülerde düşük ve T2 A görüntülerde yüksek sinyalli olan kitle yoğun heterojen kontrast tutmaktaydı. Sağ orbita üst medial duvarında ve frontal kemik bazalinde defekt izlenmekte olup, kitle kısmen orbita ve anterior kranial fossa içerisine uzanmaktaydı. Sinonazal yerleşimli malign fibröz nadir görülmekle birlikte, bu lokalizasyondaki tümörlerin ayırıcı tanısında akılda tutulmalıdır. Lokal nüks oranı yüksek olan malign fibröz histiositomda tedavi öncesi çevre dokulara invazyonun değerlendirilmesi önemlidir. MR inceleme bu kitlenin çevre dokulara yayılımının belirlenmesinde değerlidir. Anahtar kelimeler: Malign fibröz histiyositom, sinonazal, MRG
