Sütçocukluğu Döneminde Seyrek Görülen Bir Orbital Apse Olgusu+

Abstract

Orbita enfeksiyonu sıklıkla delici yaralanma ve göz kapağı, paranazal sinüs ve diş enfeksiyonları sonrasında gelişmektedir. Görme kaybı, kavernöz sinüs trombozu, menenjit, beyin apsesi ve ölüm olası komplikasyonlardır. Bu yazıda süt çocukluğu döneminde preseptal selülit tanısı ile yatırılan, ancak izleminde orbital apse saptanarak acil cerrahi drenaj uygulanan dokuz aylık bir erkek bebek sunulmuştur. Son üç gündür ateş, sağ gözde kızarıklık ve şişlik yakınmasıyla getirilen hastanın göz bölgesi görüntülemesinde, derialtı yağlı dokudan başlayarak superior ekstrakonal alana uzanan apse oluşumu izlendi. Çocukluk çağında sık karşılaşılan preseptal selülitte, uygun antibiyotik tedavisine cevap vermeyen olgularda, görüntüleme yöntemlerinin önemi vurgulanmıştır. A Rare Case of Orbital Abscess in Infancy

Abstract

Orbital abscess usually occurs as a result of penetrating trauma and infections of eyelid, paranasal sinuses and dental structure. Vision loss, cavernous sinus thrombosis, meningitis, brain abscess and even death are possible complications. We herewith present a nine-month-old male infant who was hospitalized with the presumed diagnosis of preseptal cellulitis, but finally diagnosed as orbital abscess and underwent emergent surgical drainage. He was admitted with the complaint of fever, edema and redness on right eye for the last three days. Imaging of the orbital area revealed a subcutaneous abscess protruding into superior extraconal area. We would like to emphasize the importance of sophisticated imaging techniques in this common childhood entity, preseptal cellulitis, in case of unresponsiveness despite appropriate antibiotherapy.
