Fluconazole in Neonatal Disseminated Candidiasis

Abstract

As there is continual progress in neonatal intensive care units candidemia is becoming an increasing common problem Nine cases of systemic neonatal candidiasis were diagnosed over a 2 year period in our neonatal intensive care unit Seven infants weighed less than 1500 grams The Candida species isolated from blood urine or cerebrospinal fluid were Candida parapsilosis in five 56 Candida albicans in 3 33 cases and Candida pseudotropicalis in one 11 case Antifungal susceptibility of the isolates to amphotericin B and fluconazole was determined A good correlation was found between in vitro susceptibility and in vivo activity of these antifungal agents Eight of our patients received fluconazole with good clinical and mycological response One patient with high minimal inhibitory concentration level to fluconazole gt;100 nbsp;ug ml did not respond to fluconazole and the treatment was continued with amphotericin B Based on our observation we conclude that fluconazole is a good alternative to amphotericin B and identifying the Candida species and determining the susceptibility may be useful in planning the treatment of neonatal candidemia Key words: Neonate Systemic Candidiasis Fluconazole
