Olası Q ateşi:Dört olgunun sunumu

Abstract

Q ateşi, Coxiella burnetii’ye bağlı gelişen zoonotik bir hastalıktır. Hastalık genellikle asemptomatik seyrederken atipik pnömoni, hepatit, grip benzeri tablolar görülebilmektedir. Bu çalışmada kliniğimizde olası Q ateşi tanısıyla takip edilen olguların sunulması amaçlandı. Çalışmaya Nisan 2019-Kasım 2019 tarihleri arasında Enfeksiyon Hastalıkları ve Klinik Mikrobiyoloji polikliniğine ateş, üşüme, titreme, öksürük şikayetleriyle başvuran dört olgu dahil edildi. Fizik muayenelerinde akciğerde ralleri olan olguların akciğer görüntülemesinde belirgin infiltrasyonları yoktu ve atipik pnömoni olarak değerlendirildi. Bir olguda ise nefes darlığı şikayeti olması üzerine kardiyoloji bölümüne konsülte edildi ve transtorasik ekokardiyografide perikardit saptandı. Olgularımızın tamamına serolojik tetkikler sonucuna göre olası Q ateşi tanısı kondu. Doksisiklin veya levofloksasin tedavileri verilen olgularımız şifa ile sonuçlandı. Hayvancılığın yoğun olarak yapıldığı yerlerde ateş, üşüme, titreme, öksürük gibi şikayetlerle başvuran, atipik pnömoni olarak değerlendirilen hastalarda Q ateşi akılda tutulması gereken hastalıklardan biridir.

Probable Q fever: presentation of four cases

Abstract

Q fever is a zoonotic disease caused by Coxiella burnetii. While the disease is usually asymptomatic, atypical pneumonia, hepatitis and flu like syndromes can be seen. In this study, it was aimed to present the cases followed up in our clinic with the diagnosis of probable Q fever. The study included four cases who admitted to the Infectious Diseases and Clinical Microbiology outpatient clinic between April 2019 and November 2019 with complaints of fever, chills, and cough. Cases with crackles in the lung on physical examination didn’t have significant on infiltrates on chest X-ray and were considered as atypical pneumonia. In one case, the cardiology department was consulted due to the complaint of shortness of breath, and pericarditis was detected on trans-thoracic echocardiography. Probable Q fever was diagnosed in all of our cases based on the results of serology. Our cases who given doxycycline or levofloxacin treatments resulted in cure. Q fever is one of the diseases that should be kept in mind in patients who present with complaints such as fever, chills, cough in areas where livestock is intensively made and who are evaluated as atypical pneumonia.
