MATÜR KİSTİK TERATOM VE ENDODERMAL SİNÜS TÜMÖRÜ BİRLİKTELİĞİ – OLGU SUNUMU

Abstract

Giriş: Daha önceki literatürlerde vücudun çeşitli bölgelerinde [over (1, 2), nazofarinks (3), retroperiton bölgesi (4, 5) ve safra kanalı (6)] endodermal sinüs tümörünün aynı bölgede bulunan matür kistik teratom zemininden gelişebileceği rapor edilmiştir. Olgu: Yirmiyedi yaşında, gravida 2, parite 1 olan hasta, karın ağrısı ve karında şişlik şikayetleriyle hastanemiz onkoloji kliniğine başvurdu. Ultrasonografide, sağ ovarian 16X20 cm ve sol ovarian 4X6 cm lik kitleler saptandı. Tümör belirteçleri Ca 19.9: 10 U/ml, Ca 125: 7.52 U/ml, α-feto protein: 45850 IU/ml ve CEA: 2 olarak bulundu. Hastamıza histerektomi, bilateral salfingo-oferektomi, appendektomi ve pelvik lenfadenektomi ameliyatları yapıldı. Endodermal sinüs tümörünün peritona metastaz yaptığı gözlendi. Hastaya kemoterapi (BEP: bleomisin, etoposid, sisplatin) ve hormon replasman tedavisi verildi. Tartışma: Daha önceki literatürlerde, endodermal sinüs tümörünün matür kistik teratom zemininden gelişebileceği yayınlanmıştı. Biz burada, araştırdığımız kadarıyla literatürde ilk kez, farklı overlerde eş zamanlı olarak gelişen endodermal sinüs tümörü ve matür kistik teratom vakası sunduk. Tüm bu veriler bu iki tümörün patofizyolojisinde ortak nedenlerin olabileceğini düşündürmektedir

OVARIAN ENDODERMAL SINUS TUMOR WITH CONTRALATERAL MATURE CYSTIC TERATOMA: A CASE REPORT

Abstract

Introduction: It has been reported in previous literature that mature cystic teratoma might later develope into endodermal sinus tumor at different sites of body including ovary, nasopharynx, retroperitoneal region and common bile duct. Case: A 27-year-old women G2 P1 attended to our oncology clinic with a chief complaint of abdominal pain and mass. Ultrasonography revealed right (16x10 cm) and left (4x6 cm) ovarian masses. Tumor markers were reported as Ca 19.9: 10 U/ ml, Ca 125: 7.52 U/ml, α-feto protein: 45850 IU/ml and CEA: 2. The patient underwent hysterectomy and bilateral salphingooophorectomy, pelvic lymph node dissection and appendectomy. Peritoneal metastasis of endodermal sinur tumor was present. The patient received chemotherapy (BEP: bleomycin, etoposide, cysplatin) and hormone replacement theraphy. She is healthy to date. Conclusion: To our knowledge, this is the first case where endodermal sinus tumor is not arising from a mature cystic teratoma; however, they are present concurrently.
