İlaç Hatalarının Önlenmesinde Hemşirenin Rolü: Kanıta Dayalı Yaklaşımlar

Abstract

İlaç hataları, sağlık hizmetlerinde ciddi bir hasta güvenliği sorunu oluşturmaktadır. Hemşireler, ilaç yönetim sürecinde kritik bir rol üstlenerek hata önleme mekanizmasının merkezinde yer almaktadır. Herhangi bir ilaç hatasının ilk istemden hastaya uygulamaya kadar olan tüm ilaç yönetim süreçlerinde hastaya zarar vermeden önce önlenebilir olduğu bilinmektedir. Hemşireler ilaç uygulama aşamalarındaki süreçte son güvenlik kontrolünü sağlarlar ve hastalar için hataların önlenmesinde önem teşkil ederler. Bu derlemenin amacı, ilaç hatalarının önlenmesinde hemşirelerin rollerini kanıta dayalı yaklaşımlar ışığında değerlendirmektir. Literatür verileri, sistemsel, bireysel ve çevresel faktörlerin yanı sıra, teknoloji kullanımı, sürekli eğitim, etkili iletişim ve hata raporlama sistemlerinin ilaç hatalarının önlenmesinde önemli katkılar sağladığını göstermektedir. Özellikle elektronik ilaç yönetim sistemleri, barkod ve radyo frekansı tanımlama teknolojileri gibi dijital çözümler hata oranlarını azaltmada etkili olmuştur. Ayrıca, hasta eğitimi ve multidisipliner ekip çalışması hemşirelerin hata önleyici rollerini güçlendirmektedir. Bu çalışma, hemşirelerin ilaç yönetimi süreçlerinde güvenliği artırmaya yönelik kanıta dayalı stratejilere odaklanmakta ve klinik uygulamalara yönelik önerileri sunmaktadır.
