TÜRKİYE'DE KALÇA ARTROPLASTİLERİNİN SAĞLIK POLİTİKASI AÇISINDAN DEĞERLENDİRİLMESİ

Abstract

Bu çalışmanın amacı, kalça artroplastisinde revizyon yükünü, revizyon ameliyatlarındaki artış oranını ve revizyon nedenlerini belirlemektir. Türkiye'de kamu hastanelerinde 2013-2017 yılları arasında yapılan primer ve revizyon TKA'lar retrospektif olarak incelendi. Primer ve revizyon TKA uygulanan hastalar demografik ve klinik özellikleri açısından ğerlendirilmiştir. Beş yıllık bir süre boyunca toplam 50.647 (7.472 revizyon ve 43.175 primer) işlem değerlendirildi. Kalça artroplastilerinin toplam sayısı yıllar içinde artmıştır. Hastaların %84'ü 50 yaş ve üzerindeydi ve %63,6'sı kadındı. İmplantın mekanik komplikasyonları ve implantla ilişkili her enfeksiyon, revizyon olasılığını yaklaşık 77 kat artırır. Türkiye'de birincil ve revizyon THA sayıları her yıl artmaktadır. Türkiye'de revizyon THA oranları yıllar itibariyle düşüş göstermektedir.

EVALUATION OF HIP ARTHROPLASTIES IN TÜRKİYE IN TERMS OF HEALTH POLICY

Abstract

The aim of this study is to determine the revision burden, the rate of increase in revision surgeries, and the reasons for revision in a hip arthroplasty. Primary and revision THAs, which were performed between 2013 and 2017 in public hospitals in Turkiye, were investigated retrospectively. Patients who underwent primary and revision THAs were examined in terms of their demographic and clinical characteristics. A total of 50,647 (7,472 revisions and 43,175 primary) procedures, were evaluated over a five-year period. The total number of hip arthroplasties has increased over the years. 84% of patients were 50 or older and 63.6% were women. Mechanical complications of the implant and every implant-related infection increase the probability of revision by approximately 77 times. The primary and revision THA numbers in Turkey are increasing every year. Revision THA rates have been decreasing over the years in Turkiye.
